Treatment of essential hypertension and heart failure (left ventricular dysfunction)
This medication is used to control blood pressure in patients with essential hypertension. Additionally, it helps improve symptoms of heart failure in patients with impaired left ventricular systolic function who require adjunct therapy with ACE inhibitors or who are intolerant to ACE inhibitors.
For essential hypertension, the initial and maintenance dose is recommended as 8-16 mg of Candesartan Cilexetil taken orally once daily. If blood pressure is not sufficiently controlled after four weeks of treatment at 16 mg once daily, the dose may be increased to a maximum of 32 mg once daily. For patients with reduced effective blood volume or renal impairment, the initial dose starts at 4 mg, while patients with liver disease should start at 2 mg once daily.
Safety and efficacy in children and adolescents under 18 years of age have not been established.
This medication can cause serious harm to the fetus, especially in the second and third trimesters of pregnancy, so it should be discontinued immediately if pregnancy is confirmed. This medication should not be taken by individuals allergic to this drug or its ingredients, women who are pregnant or may become pregnant, those who are breastfeeding, and individuals with severe liver disease or bile obstruction. Patients with hereditary angioedema or a history of angioedema while using ACE inhibitors or angiotensin II receptor antagonists, as well as those with primary hyperaldosteronism, should also not take this medication. Persons with genetic disorders such as lactase deficiency, Lapp lactase deficiency, or glucose-galactose malabsorption should avoid using this drug. Elderly patients, those with hyperkalemia, patients with liver disease, individuals with certain heart valve diseases or cardiomyopathy, and those with ischemic heart disease or cerebrovascular disorders should use this medication with caution. Patients with impaired kidney function or those who have received a kidney transplant, as well as adolescents and children under 18, should be cautious. Side effects may include dizziness, back pain, and respiratory infections, and rarely serious blood disorders, liver dysfunction, muscle pain, or kidney damage may occur. Particularly in heart failure patients, hypotension, hyperkalemia, and worsening kidney function may occur, thus regular blood tests are necessary. This medication should be taken with caution if used with other antihypertensives as the blood pressure-lowering effect may be increased, and particular caution is recommended when used in conjunction with potassium-sparing diuretics or potassium supplements, as this can dangerously raise serum potassium levels, warranting careful monitoring. Women who are pregnant or planning to become pregnant should not take this medication and breastfeeding women are advised to discontinue breastfeeding when taking this medication. In case of overdose, symptoms such as hypotension, dizziness, and rapid heartbeat may occur, and immediate consultation with a healthcare provider is necessary, as this drug is not removed by hemodialysis, thus requiring appropriate emergency management. Keep out of reach of children, and avoid transferring the medication to other containers as it may affect product quality. Caution is advised as dizziness may occur when driving or operating machinery.
