This medication effectively controls blood pressure using either amlodipine or olmesartan medoxomil as monotherapy for patients with essential hypertension. These drugs help treat hypertension, stabilize blood pressure, and prevent complications. This medication is used when blood pressure does not sufficiently lower with monotherapy. Therefore, it plays a critical role in managing essential hypertension, improving blood pressure control with consistent usage.

How to take

Adult dose

This medication is administered once daily with one tablet, irrespective of meals, taken with water. It is recommended to take it at the same time each day (e.g., in the morning). The dosage may be adjusted to 5/20 mg, 10/20 mg, 5/40 mg, or 10/40 mg based on blood pressure control. The maximum dosage is 10/40 mg. Patients with renal impairment, liver impairment, or elderly patients require dosage adjustment and monitoring.

Precautions

This medication can cause severe harm or death to the fetus and newborn when administered to pregnant women in the second and third trimesters, so it must be immediately discontinued if pregnancy is confirmed. Patients who are allergic to this medication's active ingredients or related drugs, pregnant or nursing women, those with severe liver function impairment, patients with severe aortic stenosis, patients with biliary obstruction, patients undergoing renal dialysis, patients in shock, certain diabetic patients, and patients with severe renal impairment should not be administered this medication. In hypotensive patients with insufficient fluid or salt levels, initial administration may induce symptoms of hypotension, so monitoring their condition closely is essential; if hypotension occurs, the patient should be laid down and normal saline may be administered as needed. This medication works by dilating blood vessels to lower blood pressure, thus it should be administered cautiously in patients with severe aortic stenosis, obstructive coronary artery disease, or congestive heart failure. Patients with impaired renal function may experience further deterioration of their renal condition with this medication, necessitating regular monitoring of kidney function. Those with liver function impairment should also exercise caution when taking this medication. During treatment, side effects such as dizziness, headache, hypotension, edema, dyspepsia, skin rash, and others may occur, and patients should consult healthcare professionals if any unusual symptoms arise. It is recommended that women who are pregnant or planning to become pregnant switch to safer alternative treatments confirmed for their safety, and this medication should be discontinued as soon as pregnancy is confirmed. Nursing mothers should be aware that this medication may pass into breast milk and can cause adverse reactions in infants; thus, nursing should be stopped or the medication should be discontinued during breastfeeding. Safety and efficacy have not been established for pediatric patients under 18 years of age, hence use in this group is not recommended. Elderly patients may have reduced renal and liver function, thus starting at a lower dosage and closely monitoring their condition is necessary. In cases of overdose, hypotension and cardiac arrhythmias may occur; hence, blood pressure and cardiac function must be carefully monitored, and immediate treatment must be initiated if needed. Taking this medication with grapefruit juice may enhance its blood pressure-lowering effects, therefore, it is advisable to avoid grapefruit juice consumption. There may be interactions when combined with other medications, so it is crucial to inform healthcare professionals of any medications currently being taken. This medication should be kept out of reach of children and stored in a cool, dry place away from direct sunlight.
