Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), and reduction of risk of death after myocardial infarction
This medication is used to effectively control blood pressure in patients with essential hypertension. It is also effective for patients with heart failure intolerant to ACE inhibitors, particularly those with moderate to severe conditions (NYHA class II to IV). It helps reduce the risk of death in patients with impaired left ventricular function following a myocardial infarction. Thus, this drug is used not only for the management of hypertension and alleviation of heart failure symptoms but also for improving survival rates after a myocardial infarction.
1. Essential hypertension: The recommended dose is 80 mg orally once daily as valsartan. If blood pressure is not adequately controlled, the dose may be increased to 160 mg once daily, and if necessary, it can be further increased to a maximum of 320 mg once daily. Elderly patients (aged 75 years and above) should start at 40 mg once daily. Patients with renal impairment, fluid depletion, or liver impairment require dose adjustments, primarily starting at 40 mg once daily.
Safety and efficacy in children and adolescents under 18 years of age have not been established, thus its use is not recommended.
This drug can cause serious harm to the fetus and newborn when taken during pregnancy, especially in the second and third trimesters, so it should be discontinued immediately upon confirmation of pregnancy. Patients with allergic reactions to this drug, severe liver dysfunction, severely impaired kidney function, women of childbearing age, and nursing mothers should not take this medication. Certain patients, such as the elderly, those with hyperkalemia, and those with mild to moderate liver dysfunction, should use this medication cautiously as there is a risk of dangerously low blood pressure; therefore, it should be started at a low dose. While taking this medication, side effects such as headache, dizziness, fatigue, dyspepsia, and cough may occur, and in rare cases, serious adverse reactions like angioedema, renal impairment, or hepatitis may happen, so any symptoms should be discussed with medical professionals immediately. If blood pressure drops sharply, there may be instances of fainting or a drop in consciousness, so if these symptoms appear, the medication should be discontinued, and appropriate treatment should be sought. This medication can increase potassium levels, so when taken with potassium-sparing diuretics or potassium supplements, blood potassium levels should be monitored closely. Interactions may occur when taken with other antihypertensives, diuretics, non-steroidal anti-inflammatory drugs (NSAIDs), so it is crucial to inform healthcare providers or pharmacists. This medication may cause dizziness or drowsiness while driving or operating machinery, so caution is advised. It is preferable not to take this medication during pregnancy or breastfeeding, and if it must be taken, a doctor should be consulted. Safety and efficacy in children and adolescents have not been sufficiently established, so its use is not recommended. In case of overdose, severe hypotension and decreased consciousness may occur, so immediate medical attention is needed.
