Prevention of thrombotic cardiovascular events (such as myocardial infarction) and stroke
This medication is effective in reducing the incidence of thrombotic cardiovascular events, namely death from cardiovascular causes, myocardial infarction, and stroke when used in conjunction with aspirin. It is particularly beneficial for adult patients with acute coronary syndrome or those at high risk of thrombotic cardiovascular events with a history of myocardial infarction over one year. It is also used to reduce the risk of myocardial infarction or stroke in patients with type 2 diabetes who have previously undergone percutaneous coronary intervention for coronary artery disease. Additionally, it contributes to the reduction of stroke incidence when used with aspirin in patients with acute ischemic stroke or high-risk transient ischemic attacks. Thus, this drug has a positive effect on the prevention and management of various thrombotic cardiovascular events.
This drug should not be used concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer a single 180mg loading dose, followed by 90mg twice daily. Treatment is recommended for 12 months unless discontinuation is clinically necessary. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome at increased risk of bleeding who have undergone percutaneous coronary intervention (PCI), monotherapy with this drug (discontinuation of aspirin) may be considered after 3 months of concomitant treatment with this drug and aspirin. (See 3. Use with caution in the following patients.) Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - Patients who received this drug 90mg or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this drug at 60mg. - Treatment with this drug may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuation of a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). - The safety and efficacy of this drug were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - For patients with acute coronary syndrome, refer to the dosage and administration for that condition. - Data on the safety and efficacy of this drug for treatment exceeding 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer a single 180mg loading dose, followed by 90mg twice daily for 30 days. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This drug is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this drug into a fine powder, disperse it in half a glass of water, and drink the mixture. Rinse the cup with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the tube with water after administration. When switching from another antiplatelet agent to this drug, start this drug 24 hours after the last dose of the previous drug. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This drug has not been studied in patients with severe hepatic impairment, and experience with its use in patients with moderate hepatic impairment is limited.
Safety and efficacy have not been established in pediatric patients younger than 18 years of age (see the 'Other' section).
This medication prevents platelets from clumping together to form clots, thereby reducing the risk of thrombus formation within blood vessels. However, due to the bleeding risk, it should not be used in patients with pathological active bleeding or a history of intracranial hemorrhage. If surgery is planned, this medication should be discontinued at least 5 days prior, and any symptoms of bleeding should be reported to healthcare professionals immediately. It should not be administered to patients with hypersensitivity to this drug or severe liver dysfunction, nor in combination with certain medications. It should be used with caution in patients at high risk of bleeding or those taking drugs that may cause bleeding, as platelet transfusions or some hemostatic treatments may be ineffective. Patients with symptoms of bradycardia, shortness of breath, or a history of asthma or chronic obstructive pulmonary disease should use it with caution. Pregnant or breastfeeding women should avoid using this drug or consult healthcare professionals before making a decision. It is not recommended for children under 18 as safety and efficacy have not been established. Elderly patients or those with decreased kidney function should have regular kidney function tests, and dose adjustments are generally not necessary. This medication may interact with other drugs, so all medications being taken should be disclosed to healthcare providers. Overdose may increase the risk of bleeding, and there is currently no known antidote, necessitating symptomatic treatment as needed. Medications should be stored out of reach of children and kept in their original containers to maintain quality. During treatment with this medication, dizziness or confusion may occur, necessitating caution when driving or operating machinery.
