This medication is used to help control blood sugar levels in patients with Type 2 diabetes. It is particularly prescribed when adequate blood sugar control is difficult with monotherapy using either metformin or pioglitazone. It is also used as a substitute for combination therapy with metformin and pioglitazone. Management of Type 2 diabetes should occur in conjunction with nutritional counseling, weight loss, and physical activity; such lifestyle improvements are crucial for enhancing and maintaining the effects of the medication. Therefore, this medication is used to improve blood sugar control in conjunction with diet and exercise.

Dosage and Administration

Adult dosage

The initial dose of this medication should be based on the patient's current treatment regimen. Start with one tablet per day, assessing the appropriateness of the treatment response, and gradually adjust the dosage. This medication should generally be divided and taken with meals to reduce gastrointestinal adverse reactions associated with metformin. Sufficient time is needed to assess the treatment response, and changes in HbA1C should be evaluated over a period of 8-12 weeks.

Precautions

This medication helps control blood sugar levels. However, caution should be exercised for the rare side effect known as lactic acidosis while taking this medication. Lactic acidosis is a serious condition that occurs when there is an excessive accumulation of lactic acid in the body, and symptoms may include difficulty breathing, muscle pain, abdominal pain, and fatigue. If these symptoms occur, consult a physician immediately. Additionally, this medication should not be administered to patients with impaired kidney function or severe heart failure. Given that diminished kidney function can lead to drug accumulation in the body, this increases the risk of side effects. Regular kidney function testing is therefore essential. Patients with impaired liver function should also avoid using this medication, as compromised liver processing of drugs can aggravate side effects. Excessive alcohol intake may affect lactate metabolism when combined with this medication, making it risky; thus, avoidance of alcohol consumption is recommended. This medication may impose a burden on the heart, and patients with heart failure symptoms should be closely monitored. If symptoms of heart failure such as weight gain, swelling, or shortness of breath arise, medical personnel should be informed immediately. Patients with a history of bladder cancer or those with active bladder cancer should not use this medication and should report any new or worsening bladder-related symptoms to their doctor. Women who are pregnant, could become pregnant, or are breastfeeding should not take this medication. While using this medication, symptoms of hypoglycemia, in which blood sugar levels drop too low, may also occur, so caution is warranted when skipping meals or engaging in vigorous exercise. This is especially critical when taking other glucose-lowering agents concurrently. Elderly patients may be at risk for decreased kidney function, so careful dosage adjustments and regular monitoring of kidney function are advised during administration of this medication. If surgery or radiographic examinations using contrast agents are planned, the use of this medication should be temporarily halted, and it should only be resumed after confirming normal kidney function. While taking this medication, it is important to have regular blood sugar, hematological, and liver function tests to ensure there are no abnormalities.
