Summary

Main use
Prevention of thrombotic cardiovascular events (such as myocardial infarction) and stroke
Who takes it
Adults · Children
Adult dose
This medication should not be used in combination with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg once, followed by 90mg twice daily. Treatment is recommended for 12 months unless discontinuation is clinically necessary. Patients taking this medication should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome who are at increased risk of bleeding and have undergone percutaneous coronary intervention (PCI), monotherapy with this medication (discontinuation of aspirin) may be considered after 3 months of concomitant treatment with this medication and aspirin. (See 3. Use with caution in the following patients.) Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this medication should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - Patients who were taking this medication at 90mg or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this medication at 60mg. - Treatment with this medication may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuation of a prior ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). - The safety and efficacy of this medication were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this medication should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - For patients with acute coronary syndrome, refer to the relevant dosage and administration instructions. - Data on the safety and efficacy of this medication for treatment lasting longer than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg once, followed by 90mg twice daily for 30 days. Patients taking this medication should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended for patients with acute ischemic stroke or high-risk transient ischemic attack. This medication is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this medication into a fine powder, disperse it in half a glass of water, and drink it immediately; rinse the cup used for administration with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the tube with water after administration. If switching from another antiplatelet agent to this medication, start this medication 24 hours after the last dose of the previous medication. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is required in patients with renal impairment. No dose adjustment is required in patients with mild hepatic impairment. This medication has not been studied in patients with severe hepatic impairment, and there is limited treatment experience in patients with moderate hepatic impairment.
Child dose
Safety and efficacy have not been established in pediatric patients under 18 years of age (see the “Other” section).

This medication helps reduce the incidence of thrombotic cardiovascular events when used in combination with aspirin in adult patients. It lowers the incidence of death due to cardiovascular events, myocardial infarction, and stroke in high-risk patients with acute coronary syndrome or a history of myocardial infarction for at least 1 year. Additionally, it reduces the incidence of myocardial infarction or stroke in patients with type 2 diabetes who have undergone PCI for coronary artery disease. It is effective in preventing strokes when used in combination with aspirin in patients with acute ischemic stroke or high-risk transient ischemic attacks. This medication is used to lower the risk of thrombotic cardiovascular events.

Safety and efficacy have not been established in pediatric patients under 18 years of age (see the “Other” section).

Precautions

This medication prevents blood clotting, preventing thrombosis. Since it can increase the risk of bleeding due to impaired blood coagulation, it should not be used in patients with active bleeding or a history of intracranial hemorrhage. If surgery is planned, this medication must be discontinued at least 5 days before the procedure, and any symptoms of bleeding should be reported to healthcare personnel immediately. It is contraindicated in patients with an allergy to this medication, severe liver dysfunction, or in combination with certain strong medications. Patients at high risk of bleeding, or those with recent trauma, surgery, or coagulopathy, should use this medication with caution, especially if they are taking other drugs that may induce bleeding. In patients who have undergone cardiac procedures, the risks of bleeding and thrombosis should be carefully considered when discontinuing aspirin while using this medication. This medication may cause bradycardia; patients with bradycardia or those taking related medications should exercise caution. Symptoms of shortness of breath may occur, and patients with asthma or chronic obstructive pulmonary disease should be particularly cautious when taking this medication. Additionally, this medication may affect kidney function, so elderly patients or those with kidney dysfunction must undergo regular kidney function tests. Patients with a history of gout or hyperuricemia should also take this medication cautiously. This medication can interact with multiple other drugs, so patients taking other medications must inform their physician or pharmacist. Women who are pregnant or breastfeeding should avoid taking this medication and appropriate contraception is recommended. This medication is not recommended for children under 18 years of age due to insufficient evidence of safety and efficacy. In cases of overdose, the risk of bleeding may increase, and as there is no specific antidote, immediate medical treatment should be sought. During treatment with this medication, symptoms such as dizziness or confusion may occur, so care should be taken when driving or operating machinery. This medication inhibits platelet function, thus helping to prevent cardiovascular events such as myocardial infarction and stroke.
