Treatment of essential hypertension and heart failure (including ACE inhibitor intolerance), risk reduction of mortality after myocardial infarction
This drug is used for the treatment of essential hypertension and heart failure patients without ACE inhibitor intolerance (NYHA class II to IV). It also helps to reduce the risk of death in stable patients showing symptoms or left ventricular dysfunction after myocardial infarction. Therefore, this medication is effectively used for blood pressure control, improvement of heart failure symptoms, and improvement of prognosis after myocardial infarction.
1. Essential Hypertension - Adults: It is recommended to take 80 mg orally once a day. - If blood pressure control is insufficient, the dose may be increased to 160 mg once a day, and up to a maximum of 320 mg may be administered. - Elderly patients (75 years and older): Start with 40 mg once daily. - Patients with kidney impairment (creatinine clearance 10-20 mL/min): Start with 40 mg once daily. 2. Heart Failure Adults: The starting dose is recommended to be 40 mg taken orally twice a day, and can be increased to 80 mg or 160 mg once per dose. When used in combination with diuretics, the dose of the diuretic should be considered for reduction. The maximum daily dose is 320 mg in divided doses. Triple combination with ACE inhibitors and beta-blockers is not recommended. 3. Risk Reduction of Mortality after Myocardial Infarction Adults: Treatment can begin within 12 hours after the onset of myocardial infarction. The recommended starting dose is 20 mg, taken twice daily, increasing to 40 mg, 80 mg, or 160 mg as tolerated over several weeks. Consider dose reduction if symptoms of hypotension or renal failure occur. This drug can also be given to patients being treated with other therapies after myocardial infarction (e.g., thrombolytics, acetylsalicylic acid, beta-blockers, and statins). However, combination with ACE inhibitors is not recommended. 4. Patients with Kidney Impairment If creatinine clearance is 10 mL/min or more, no dose adjustment is necessary; however, there is no experience with patients with creatinine clearance of less than 10 mL/min and dialysis patients, so this drug should not be administered. 5. Patients with Liver Impairment For patients with mild to moderate liver impairment, the total daily dose of this drug should not exceed 80 mg. If the dose exceeds 80 mg a day, the benefits of the treatment must be weighed against its risks. This drug should not be given to patients with severe liver impairment, cirrhosis, or biliary obstruction. 6. Children The safety and efficacy have not been established in children and adolescents under 18 years of age.

This drug can cause serious damage to the fetus if taken during pregnancy, especially during the second and third trimesters, so it should be discontinued immediately if pregnancy is confirmed. This drug should not be used in patients with certain diseases, such as severe liver impairment, severe kidney impairment, a history of angioedema, pregnant women, or nursing mothers. Elderly patients or those with hyperkalemia, mild to moderate liver impairment, or cardiovascular disease should be particularly careful when using this drug, and the risk of hypotension or worsening kidney function should always be monitored. Side effects such as headache, dizziness, fatigue, cough, and diarrhea may occur during treatment with this drug; consult a healthcare professional if symptoms are severe or persistent. In patients with heart failure or after myocardial infarction, kidney function and electrolyte status should be monitored regularly and appropriate action should be taken if abnormalities are found. This drug can interact with other medications, so caution is needed when used in combination with potassium-sparing diuretics, potassium supplements, or non-steroidal anti-inflammatory drugs (NSAIDs). Women who are pregnant or planning to become pregnant should not take this drug, and nursing mothers are advised to avoid use as well. The safety and efficacy of this drug have not been established in children and adolescents under 18 years of age, thus it is not recommended. In case of overdose, severe hypotension, decreased consciousness, or shock may occur, so seek immediate medical attention for appropriate treatment. This drug should be kept out of reach of children, and it is important to avoid transferring it to other containers.
