Prevention of thrombotic cardiovascular events in patients with acute coronary syndrome or a history of myocardial infarction
This medication helps reduce the incidence of thrombotic cardiovascular events, specifically death from cardiovascular causes, myocardial infarction, and stroke, when used in conjunction with aspirin. It is particularly effective in adult patients with acute coronary syndrome or those at high risk for thrombotic cardiovascular events after a history of myocardial infarction for over a year. It is used to improve the patient’s cardiovascular health and prevent recurrence.
Patients taking this drug should also receive a maintenance dose of aspirin (75 ∼ 150mg), unless specifically contraindicated. Do not use this drug concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). • Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation is clinically necessary. • Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. - Patients who received this drug at 90mg or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this drug at 60mg. - Treatment with this drug may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuation of a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). - The safety and efficacy of this drug were evaluated at 3 years of follow-up. Administration This drug is administered orally and may be taken with or without food. • Patients who have difficulty swallowing the tablet should crush this drug into a fine powder, disperse it in half a glass of water, and drink it immediately. Rinse the glass with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. • When switching from another antiplatelet agent to this drug, start this drug 24 hours after the last dose of the previous medication. • If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. Patients with renal impairment No dose adjustment is necessary in patients with renal impairment. There are no data on the use of this drug in patients receiving dialysis; therefore, its use is not recommended in these patients. Patients with hepatic impairment No dose adjustment is necessary in patients with mild hepatic impairment. This drug has not been studied in patients with severe hepatic impairment, and there is limited experience with its use in patients with moderate hepatic impairment.
The safety and efficacy of this drug have not been established in pediatric patients younger than 18 years.
This medication prevents blood clotting. However, it should not be used in patients with pathological bleeding or a history of intracranial bleeding due to high risks of bleeding. Patients scheduled for surgery should stop taking this medication at least 7 days prior, and should seek medical attention immediately if symptoms of bleeding occur. Patients with hypersensitivity to this medication, severe liver dysfunction, or those taking strong CYP3A4 inhibitors should not receive this drug. Careful use is required in patients with high bleeding risk, those who have recently undergone surgery, or if they have a tendency to bleed, especially in combination with other medications that may induce bleeding. Caution is especially warranted when used with warfarin as it may increase the risk of bleeding. Symptoms such as dizziness, confusion, and difficulty breathing may occur during use, and such symptoms should prompt avoidance of driving or operating machinery. Pregnant or nursing women should consult a doctor before taking this medication, and women of childbearing age should use appropriate contraception to avoid pregnancy during treatment. Safety and effectiveness in patients younger than 18 years have not been established, so it is not recommended for this age group. In case of overdose, the risk of bleeding increases, and appropriate medical treatment should be sought immediately. This drug can interact with other medications, so any current medications should be reported to healthcare providers. Especially caution is needed when co-administered with simvastatin, rosuvastatin, digoxin, or CYP3A4 inhibitors or inducers. Elderly patients can use this medication without special dosage adjustments, but regular check-ups such as kidney function tests are necessary. During treatment, symptoms of bleeding and difficulty breathing should be monitored periodically, and any abnormal symptoms should be reported to healthcare providers immediately.
