Summary

Main use
Prevention of thrombotic cardiovascular events (myocardial infarction, etc.) and stroke
Who takes it
Adults · Children
Adult dose
This medicine should not be used concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation of this medicine is clinically necessary. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome who have an increased risk of bleeding and have undergone percutaneous coronary intervention (PCI), monotherapy with this medicine (discontinuing aspirin) may be considered after 3 months of concomitant treatment with this medicine and aspirin. (See 3. Use with caution in the following patients.) Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of atherothrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients who took 90mg of this medicine or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with 60mg of this medicine. Treatment with this medicine may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). The safety and efficacy of this medicine were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. For patients with acute coronary syndrome, refer to the relevant dosage and administration instructions. Data on the safety and efficacy of this medicine when administered for more than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended for patients with acute ischemic stroke or high-risk transient ischemic attack. This medicine is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this medicine into a fine powder, disperse it in half a glass of water, and drink it immediately. Rinse the glass used for administration with half a glass of water and drink the rinse. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. When switching from another antiplatelet agent to this medicine, start this medicine 24 hours after the last dose of the previous medicine. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This medicine has not been studied in patients with severe hepatic impairment, and experience with its use in patients with moderate hepatic impairment is limited.
Child dose
The safety and efficacy of this medicine have not been established in pediatric patients under 18 years of age (see the 'Other' section).

This medication helps reduce the incidence of thrombotic cardiovascular events, namely death from cardiovascular causes, myocardial infarction, and stroke, when used in conjunction with aspirin. It is particularly useful for patients with acute coronary syndrome or those who have previously experienced a myocardial infarction, where there is a high risk of thrombotic events. It can also reduce the incidence of myocardial infarction or stroke in patients with type 2 diabetes who have undergone procedures for coronary artery disease. In patients with acute ischemic stroke or high-risk transient ischemic attack, it reduces the risk of stroke when used together with aspirin. In summary, this medication effectively improves the incidence of thrombotic cardiovascular events and stroke when combined with aspirin.

Precautions

This medication prevents blood clots by inhibiting blood coagulation. It poses a risk of bleeding; therefore, it should not be used in patients with active bleeding or a history of intracranial hemorrhage. If surgery is planned, the medication should be stopped at least 5 days prior to reduce the risk of bleeding. During the medication course, any symptoms of bleeding such as nosebleeds, gum bleeding, hematuria, or bruising should be immediately reported to a doctor. Patients at high risk of bleeding or taking other medications that may cause bleeding should use this medication cautiously. Patients who have undergone heart surgery or coronary interventions should use the drug according to their doctor's instructions, considering the risk of bleeding and thrombotic events. The medication may cause breathing difficulties, especially in patients with asthma or chronic obstructive pulmonary disease, and caution is advised. It may induce bradycardia, so patients at risk for bradycardia should consult their doctor. This medication is not administered to patients with severe liver or kidney dysfunction, and elderly patients or those with decreased kidney function should have regular kidney function tests. Women who are pregnant or breastfeeding must consult a doctor before taking this medication, and women of childbearing age should use appropriate contraception to avoid pregnancy. This medication may interact with other drugs, so inform your physician or pharmacist of any currently taken medications. Overdose may increase the risk of bleeding, and as there are no specific antidotes, appropriate medical treatment must be sought immediately. Dizziness or confusion may occur during medication use, so caution is needed when driving or operating machinery. This medication is not recommended for children under 18 years as safety and effectiveness have not been established.
