Prevention of thrombotic cardiovascular events (e.g., myocardial infarction) and stroke
This medication is used to reduce the incidence of thrombotic cardiovascular events. It is effective when used in combination with aspirin in dangerous situations such as death caused by cardiovascular complications, myocardial infarction, and stroke. It lowers the risk of thrombotic cardiovascular events in patients with acute coronary syndrome or a history of myocardial infarction. It is also beneficial for patients with type 2 diabetes who have undergone coronary intervention without a history of myocardial infarction or stroke. It reduces the incidence of stroke when used with aspirin in patients with acute ischemic stroke or high-risk transient ischemic attack. This drug plays an important role in the prevention and treatment of cardiovascular diseases.
Do not use this drug in combination with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation is clinically necessary. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome who are at increased risk of bleeding and have undergone percutaneous coronary intervention (PCI), monotherapy with this drug (discontinuing aspirin) may be considered after 3 months of concomitant treatment with this drug and aspirin. (See section 3. Patients requiring careful administration.) Patients with a history of myocardial infarction (occurring at least 1 year earlier) and at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - Patients who took 90mg of this drug or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this drug at 60mg. - Treatment with this drug may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). - The safety and efficacy of this drug were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - For patients with acute coronary syndrome, refer to the corresponding dosage and administration. - Data on the safety and efficacy of this drug for use longer than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial aspirin loading dose (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This drug is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this drug into a fine powder, disperse it in half a glass of water, and drink it immediately; rinse the glass with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. When switching from another antiplatelet agent to this drug, start this drug 24 hours after the last dose of the previous medication. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This drug has not been studied in patients with severe hepatic impairment, and experience with administration in patients with moderate hepatic impairment is limited.
Safety and efficacy have not been established in pediatric patients under 18 years of age (see “Other”).
This medication prevents thrombosis by inhibiting platelets from aggregating in the bloodstream. However, similar to other anticoagulants, it may increase the risk of bleeding, so it should not be used in patients with bleeding or a history of intracranial hemorrhage. Patients planning to undergo surgery should discontinue this medication at least 5 days prior to the procedure to minimize the risk of bleeding. It should be administered cautiously to patients at high risk of bleeding, such as those who have recently undergone surgery or have a tendency to bleed, and any signs of bleeding should be reported immediately to healthcare professionals. This medication may increase drug concentration and the risk of side effects when taken with strong medications or in patients with severely impaired liver function, so combinations should be avoided. Bradycardia may occur, so patients with heart disease should take precautions. Dyspnea may develop, especially in patients with asthma or chronic obstructive pulmonary disease (COPD). Elderly patients, those with impaired kidney function, and those with a history of hyperuricemia or gout should be closely monitored while taking this medication. This drug may interact with various medications, so be sure to inform healthcare providers if taking other drugs. It is advisable for pregnant or breastfeeding women to avoid this medication, and women of childbearing age should take appropriate contraceptive measures to avoid pregnancy while on this drug. An overdose increases the risk of bleeding, so the medication should be taken as prescribed, and any unusual symptoms should be discussed with healthcare professionals immediately. Dizziness or confusion may occur while taking this medication, so caution is needed when driving or operating machinery. Finally, the safety and efficacy of this medication have not been established in children under the age of 18.
