Treatment of major depressive disorder, anxiety disorders (e.g., panic disorder and social anxiety disorder), and obsessive-compulsive disorder
This medication is used for the treatment of various anxiety-related disorders including major depressive disorder, panic disorder (with or without agoraphobia), social anxiety disorder (social phobia), generalized anxiety disorder, and obsessive-compulsive disorder.
The safety of doses exceeding 20mg per day has not been established. Administer this drug once daily in the morning or evening, with or without food. (1) Major depressive disorder: The usual dose is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day. If increasing the dose to 20mg, allow an interval of at least 1 week before increasing the dose. In general, 2-4 weeks are needed for antidepressant effects to develop. Even after symptoms have resolved, drug treatment for at least 6 months is necessary to ensure a therapeutic effect. (2) Panic disorder with or without agoraphobia: The recommended initial dose is 5mg once daily for 1 week, followed by an increase to 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day. Maximum effects appear after approximately 3 months. The safety and efficacy of long-term use of this drug for more than 10 weeks in the treatment of panic disorder have not been systematically studied. (3) Social anxiety disorder: The usual dose is 10mg once daily. In general, 2-4 weeks are needed for symptom improvement. Thereafter, depending on the patient's response, the dose may be reduced to 5mg per day or increased to a maximum of 20mg per day. Social anxiety disorder is a chronic condition; continued treatment for 12 weeks is recommended to ensure a therapeutic effect. Long-term treatment in patients who respond has been studied for 6 months, and should be considered on an individual patient basis to prevent relapse. (4) Generalized anxiety disorder: The initial dose is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day after at least 1 week of treatment. A long-term treatment study was conducted in patients who responded to 12 weeks of open-label treatment, in which 20mg per day was administered for at least 6 months. (5) Obsessive-compulsive disorder: The usual dose is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg per day. A long-term treatment study was conducted in patients who responded to 16 weeks of open-label treatment, in which 10mg or 20mg per day was administered for at least 24 weeks. Obsessive-compulsive disorder is a chronic condition, and treatment should continue for a sufficient period to ensure resolution of symptoms. When this drug is used long term, the benefit and dose should be reassessed regularly for each patient. 2) Patients with renal impairment Dose adjustment is not necessary in patients with mild or moderate renal impairment. Caution should be exercised in patients with severe renal impairment (CLCR < 30㎖/min). 3) Patients with hepatic impairment An initial dose of 5mg once daily for the first 2 weeks is recommended. Depending on the patient's response, the dose may be increased to 10mg per day. 4) Discontinuation of treatment When discontinuing this drug, the dose should be gradually reduced over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. 5) Switching from MAO inhibitors used to treat psychiatric disorders When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this drug, allow an interval of at least 14 days. Conversely, at least 14 days must elapse after discontinuing this drug before starting an MAO inhibitor to treat psychiatric disorders. 6) Other MAO inhibitors, such as linezolid or methylene blue It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this drug. If no alternative to linezolid or intravenous methylene blue is available and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome in a particular patient, this drug may be discontinued immediately and linezolid or intravenous methylene blue administered. The patient should be monitored for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this drug may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by a nonintravenous route (oral tablets or local injection), or with intravenous administration of this drug at 1mg/kg or less, is unclear.
Caution is needed as this medication may increase the risk of suicidal thoughts or behavior in children, adolescents, and young adults with depression or psychiatric disorders. This medication should not be used in cases of allergy to it or when taking certain other medications, especially MAO inhibitors, pimozide, or drugs that may prolong the QT interval. Patients with a history of glaucoma or those with angle-closure glaucoma should use this medication cautiously as it may dilate pupils and increase intraocular pressure. During the first 1 to 2 weeks of treatment, side effects such as anxiety, insomnia, dizziness, and nausea may commonly occur, but symptoms often alleviate over time. Patients and caregivers should closely monitor for changes in mental state, including suicidal thoughts, aggressiveness, anger, and anxiety, while taking this medication. Particularly for patients with a history of seizures or manic symptoms, there is a need for caution as this medication may worsen seizures or induce mania. Diabetic patients may experience changes in blood glucose control while taking this medication, thus requiring adjustments in insulin or antidiabetic medications. Abrupt discontinuation of this medication may lead to withdrawal symptoms such as dizziness, anxiety, and sleep disturbances; gradual tapering should be done under a doctor's guidance. Patients with heart disease or those with QT interval prolongation should undergo an electrocardiogram before starting this medication and consult immediately if symptoms of cardiac arrhythmia occur during treatment. This medication may cause serious side effects such as serotonin syndrome when used in conjunction with other serotonergic drugs; therefore, concomitant use should be discussed with a healthcare professional. Women who are pregnant or breastfeeding should carefully consider this medication's use as it may affect the fetus or newborn; discontinuation of breastfeeding is recommended if taking this medication while nursing. Older adults should start with a lower initial dose and adjust the maximum dose accordingly for safety. Caution is necessary as this medication may impair driving or machine operation. Overdosing on this medication may result in symptoms such as dizziness, tremors, nausea, and cardiac arrhythmia, necessitating immediate medical attention, and there is no specific antidote. Keep this medication out of reach of children and do not transfer it to other containers for safety.
