Treatment and progression inhibition of prostate cancer (metastatic, hormone-responsive)
This drug is used to treat patients with metastatic castration-resistant prostate cancer and is effective even in asymptomatic or mildly symptomatic cases. It is also applicable to patients who have received chemotherapy including docetaxel. In newly diagnosed patients with hormone-responsive high-risk metastatic prostate cancer, it is treated in combination with androgen deprivation therapy. This drug must be used in conjunction with prednisone. It plays a very important role in the treatment of prostate cancer.
The recommended dose for this drug is 1,000 mg (2 tablets of 500 mg) once daily, which must be taken on an empty stomach. The drug should be taken at least 2 hours after a meal and no food should be consumed for at least 1 hour after taking the drug. The tablets must be swallowed whole with water. In cases of liver dysfunction, the dose should be reduced to 500 mg (1 tablet) once daily after liver function test results show recovery.
This drug must not be administered to pregnant women or women who may become pregnant. Patients with an allergy to this drug or its components should not use it. Patients with lactose intolerance or certain genetic metabolic disorders should not take this drug. Patients taking radium-223 chloride should not use this drug. This drug can increase mineralocorticoid levels, potentially causing hypertension, hypokalemia, and fluid retention, therefore caution is required for patients with these symptoms or with cardiac diseases. Blood pressure, serum potassium levels, and fluid status should be regularly monitored. Patients with liver dysfunction or those showing signs of hepatic toxicity should use this drug cautiously and undergo frequent liver function tests. If liver enzyme levels rise significantly or liver function worsens, the drug should be discontinued immediately. Patients with a history of cardiovascular diseases also need to be cautious when taking this drug. In various clinical trials, side effects such as peripheral edema, hypokalemia, elevated liver enzymes, hypertension, and urinary tract infections have been commonly reported. These side effects can be managed with appropriate treatment and care. This drug should be taken on an empty stomach as its absorption increases with food. It may interact with other medications, especially those metabolized by the liver, so consultation with a specialist is advised when taking such medications together. The safety of this drug during pregnancy or breastfeeding has not been established, and male patients are advised to use condoms during sexual relations with fertile women. It is not for use in children. It can be used in the elderly, but careful observation based on their physical condition is necessary. In the case of overdose, discontinue the drug immediately and monitor for cardiac arrhythmias and liver function, receiving appropriate treatment. This drug should be kept out of the reach of children and stored in its original container. During the use of this drug, patients should regularly check their health status, including liver function, blood pressure, and serum potassium levels, and consult medical personnel immediately if any abnormal symptoms appear.
