Treatment of essential hypertension and reduction of the risk of cardiovascular diseases
This medicine is used for the treatment of essential hypertension. Additionally, it helps reduce the risk of myocardial infarction, stroke, and cardiovascular disease-related mortality in patients aged 55 and older who are at high risk for cardiovascular diseases, particularly those with a history of coronary artery disease, peripheral artery disease, stroke, transient ischemic attacks, evidence of end-organ damage, or high-risk diabetes. Therefore, this drug is used in the treatment of hypertension and prevention of cardiovascular diseases.
For essential hypertension, telmisartan is administered orally at a dose of 40 mg once daily with water, regardless of meals. As needed, treatment may begin at 20 mg once daily and can be increased to 80 mg once daily. Optimal blood pressure-lowering effects are typically observed 4 to 8 weeks after starting treatment. To reduce the risk of cardiovascular diseases, 80 mg is recommended to be taken once daily with water.
This medication may cause serious harm to the fetus and newborn, especially during pregnancy, and should be discontinued immediately if pregnancy is confirmed. Individuals with allergies to this drug or its ingredients, pregnant or breastfeeding women, those with severe liver dysfunction, history of hereditary angioedema, and those with diabetes or moderate to severe renal impairment should not use this medication. Elderly individuals, patients with hyperkalemia, those with liver dysfunction, cardiovascular diseases, gastrointestinal disorders, or renal vascular diseases should use this medication with caution. Various adverse reactions may occur during this treatment, including infections, blood abnormalities, allergic reactions, hypotension, liver dysfunction, and renal dysfunction, so the patient's condition should be closely monitored. If symptoms of angioedema, muscle pain, weakness, dizziness, or shock appear, medical personnel should be notified immediately. Patients with impaired renal function should have their serum potassium and creatinine levels checked regularly, as hypotension may occur if blood volume is decreased or salt is lacking. The blood-pressure-lowering effect of this medication may be enhanced when used with other antihypertensive drugs, so it is essential to consult healthcare providers regarding interactions with other medications being taken. Particularly, when used with ACE inhibitors, aliskiren, potassium-sparing diuretics, lithium, and non-steroidal anti-inflammatory drugs (NSAIDs), the risk of renal impairment, hyperkalemia, and hypotension may increase. Pregnant or potentially pregnant women should avoid taking this medication, and those who are breastfeeding are also advised to refrain from its use. Safety and efficacy have not been established for children under 18 years of age, so its use is not recommended. The elderly should use caution due to the risk of excessive blood pressure reduction leading to stroke or other complications. In case of overdose, severe hypotension, dizziness, and acute renal failure may occur, necessitating an immediate visit to a medical institution for appropriate treatment. This medication should be stored out of the reach of children, and the sealed packaging should not be opened until just before use. Care should be taken while driving or operating machinery, as fainting or dizziness may occur.
