Summary

Main use
Treatment of herpes zoster virus infection
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster infection Adults: Administer 750 mg of famciclovir orally once daily for 7 days. Take it at the same time each day. It is preferable to start treatment as soon as symptoms appear after infection. 2. Patients with renal impairment: Since the creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be undertaken with caution. The dosage for these patients is as follows. 1) Treatment of herpes zoster infection Creatinine clearance (mL/min/1.73㎡) 30~59 : 250 mg twice daily 10~29 : 250 mg once daily 2) If only the serum creatinine level is known, creatinine clearance may be calculated using the following equations. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 3) Patients with renal impairment undergoing hemodialysis: In patients receiving hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer the full dose immediately after dialysis. 3. Patients with hepatic impairment: No dose adjustment is required in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with evident chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This medication should not be used in patients who have had allergic reactions to this drug or its ingredient, penciclovir. Patients with renal function issues should exercise more caution when using this medication, and dosage adjustments may be necessary. While taking this medication, symptoms such as headache or nausea may occur, but most cases are mild. Rarely, a decrease in platelet count or leukopenia may occur, so symptoms should be monitored closely. Neurological symptoms such as dizziness, drowsiness, and confusion may appear, so it is advisable to avoid driving or operating machinery if these symptoms occur. Gastrointestinal symptoms such as nausea, vomiting, abdominal pain, and diarrhea may commonly occur. Abnormal liver function tests or, rarely, jaundice may occur, so it is necessary to periodically check liver health. Skin rashes or itching may commonly occur, and there is a rare risk of severe skin reactions. In patients with impaired renal function, acute kidney failure has been reported rarely, so renal status should be managed carefully. This drug does not cure genital herpes, so it is important to avoid sexual contact and maintain safe sexual relations if symptoms are present during treatment. There is no evidence that this drug affects the ability to drive or operate machinery, but caution should be taken in case of dizziness or drowsiness. Patients with slightly impaired liver function do not need special dosage adjustments, but in cases of severe liver dysfunction, therapeutic effects may diminish. This medication has a low likelihood of interactions with other drugs, but caution is advised when used in combination with drugs that affect renal excretion due to toxicity risks. Women who are pregnant or may become pregnant should carefully weigh the benefits and risks when using this medication. Women who are breastfeeding should use this medication only when absolutely necessary, as it may be transmitted through breast milk. It is advisable not to use this medication in children under 18 years of age, as safety and efficacy have not been established. For the elderly, dosage adjustments are generally not necessary unless renal function is decreased. In cases of overdose, appropriate treatment and dialysis may be performed to lower the drug concentration in the blood. This medication should be stored in a dry place at temperatures below 30 degrees Celsius, and kept out of reach of children. Care should be taken not to transfer to other containers, as this may cause accidents or deterioration in quality.
