Summary

Main use
Treatment and suppression of recurrence of genital herpes; treatment of herpes zoster
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster infection Adults: Administer 250 mg of famciclovir per dose orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer 250 mg of this medicine per dose orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer 125 mg of this medicine per dose orally twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine per dose orally twice daily. Discontinue treatment periodically every 6~12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Since creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be undertaken with care. The dosage for these patients is as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily Creatinine clearance (mL/min/1.73㎡) 10~29: 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose twice daily Creatinine clearance (mL/min/1.73㎡) 10~29: 125 mg per dose once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily Creatinine clearance (mL/min/1.73㎡) 10~29: 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight(kg) × (140-age)}/ {72 × serum creatinine(mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No separate dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.

Precautions

This medication should not be administered to certain patients. Patients who have had allergic reactions to this medication or its components, or those with genetic issues like lactose intolerance should not take this medication. Caution is required when using this medication for patients with impaired kidney function. During treatment with this medication, symptoms such as headache, nausea, dizziness, and drowsiness may occur, particularly drowsiness, which may be more common in the elderly. Additionally, there may be a decrease in blood cell count or skin rashes, itching, and rarely, severe skin reactions, so if symptoms occur, medical consultation is required immediately. Acute renal failure may rarely occur in patients with poor kidney function, so renal status should be closely monitored. This medication does not cure genital herpes, so it is important to avoid sexual contact and maintain safe sexual practices if symptoms are present during treatment. If dizziness or drowsiness occurs, driving or operating machinery should be avoided. Patients with mild to moderate liver dysfunction do not require special dosage adjustments, but severe liver impairment may reduce efficacy, so caution is advised. This medication has low potential for drug interactions; however, monitoring blood concentrations or toxicity may be necessary when used with specific medications. Women who are pregnant or breastfeeding should carefully consider the risks and benefits of taking this medication, as should women who may become pregnant. Children under 18 years of age are generally advised against using this medication due to undetermined safety and efficacy. Elderly patients typically do not require dosage adjustments, but dosage should be adjusted in cases of reduced kidney function. In cases of overdose, appropriate treatment along with hemodialysis may be helpful. This medication should be stored in a dry place below 30 degrees Celsius and kept out of reach of children. Additionally, transferring this medication to another container can be detrimental to maintaining quality and should be avoided.
