Improvement of atherosclerotic symptoms; prevention of thromboembolism associated with atrial fibrillation
This medication is used to improve atherosclerotic symptoms such as ischemic stroke, myocardial infarction, or peripheral artery disease. Additionally, it helps reduce the risk of death, myocardial infarction, and stroke due to cardiovascular abnormalities in patients with acute coronary syndrome, particularly those who have unstable angina or non-Q wave myocardial infarction and have undergone coronary intervention or coronary artery bypass surgery. In patients with atrial fibrillation who have vascular risk factors and for whom vitamin K antagonists are not suitable and the risk of bleeding is low, this drug reduces the risk of stroke and atherosclerotic thrombosis and thromboembolism. In summary, this medication is actively used to improve and prevent various cardiovascular diseases and associated thrombotic risks.
○ Adults 1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), start treatment on the first day with a loading dose of 300 mg once daily, followed by a maintenance dose of 75 mg orally once daily. In this case, aspirin 75 ~ 325 mg should be administered concomitantly with this medicine once daily. 3. For patients with atrial fibrillation, administer this medicine 75 mg orally once daily. In this case, aspirin 75 ~ 100 mg should be administered concomitantly with this medicine once daily. ○ Patients with renal impairment: Treatment experience in patients with renal impairment is limited. ○ Patients with hepatic impairment: Treatment experience in patients with moderate liver disease with a bleeding diathesis is limited. This medicine may be administered with or without food.
This medication is an agent that prevents platelets from aggregating, thereby preventing blood clotting. The effectiveness of this medication may be diminished in individuals who genetically have reduced function of the CYP2C19 enzyme; therefore, alternative treatment options should be considered in such cases. Moreover, co-administration with medications that increase the activity of the CYP2C19 enzyme may elevate the concentration of this drug, increasing the risk of bleeding, so it is advisable not to combine them. Patients with allergies to this drug, those who are bleeding, patients with severe liver damage, nursing women, and individuals with lactose-related genetic disorders should not take this medication. Patients at high risk of bleeding or with blood disorders, and those scheduled for surgery should use this medication with caution, and if bleeding symptoms occur, they should promptly inform their healthcare provider. The risk of bleeding may increase when this medication is taken with other anticoagulants, non-steroidal anti-inflammatory drugs, or selective serotonin reuptake inhibitors, so caution is advised. During pregnancy, there is a low risk to the fetus, but caution should be taken due to the risk of bleeding during delivery, and during breastfeeding, this drug may be secreted in breast milk, so consideration should be given to stopping breastfeeding or discontinuing the medication. The safety and efficacy of this drug in pediatrics have not been established, and caution is needed for elderly patients or those with reduced renal or hepatic function, as bleeding risk may be heightened. In cases of overdose, bleeding may worsen, and immediate medical treatment is required, as platelet transfusions may be necessary to reverse the effects of this drug. During treatment, it may take longer for bleeding to stop; thus, patients should immediately inform their doctor if bleeding persists or if there are signs of ongoing bleeding. This medication should be stored out of reach of children, and transferring it to another container should be avoided as it may cause accidents or degradation in quality.
