Treatment of overactive bladder symptoms (e.g., urinary frequency and urgency)
This medication is used to alleviate the primary symptoms of overactive bladder such as urgent urinary incontinence, frequent urination, and urinary urgency. It helps control frequent urination and the sudden urge to urinate caused by overactive bladder, reducing discomfort in daily life. By improving these symptoms, it aids in enhancing the quality of life for patients. Therefore, it is effective for the treatment of overactive bladder.
The usual recommended dose for adults (including elderly patients) is 5mg once daily. If necessary, the dose may be increased to a maximum of 10mg once daily. This drug may be taken with or without food and should be swallowed whole with water. No dose adjustment is necessary in patients with mild or moderate renal impairment (creatinine clearance>30ml/min). Use with caution in patients with severe renal impairment (creatinine clearance≤30ml/min) and do not exceed 5mg once daily. No dose adjustment is necessary in patients with mild hepatic impairment. Use with caution in patients with moderate hepatic impairment (Child-Pugh Class B) and do not exceed 5mg once daily. For patients receiving ketoconazole or another potent CYP3A4 inhibitor (e.g., ritonavir, nelfinavir, itraconazole), limit the maximum daily dose to 5mg.
This medication should not be used in patients with urinary retention or serious gastrointestinal diseases such as toxic megacolon. Patients with myasthenia gravis, narrow-angle glaucoma, and those allergic to this medication or its components should also avoid it. It is not recommended for patients undergoing dialysis or those with severely impaired liver or kidney function. Patients with genetically-based lactose intolerance should not take this medication. Caution is advised when using this medication in patients at risk of bladder outlet obstruction or those with gastrointestinal obstruction or reduced gastrointestinal motility. Patients with severe kidney or liver impairment should not exceed the maximum daily dosage and should exercise caution when taking it with certain potent medications. Patients taking medications that can induce or exacerbate gastroesophageal reflux or esophagitis should be carefully monitored. Patients with autonomic neuropathy should also be cautious when using this medication. The most common side effects of this medication include dry mouth and constipation, and symptoms such as blurred vision, urinary retention, and dry eyes may also occur. Severe gastrointestinal side effects may arise with high doses, so caution is advised. There may be cases where the medication is discontinued due to side effects, with dry mouth being the most common reason. Caution is warranted as this medication can cause cardiac rhythm disturbances in patients with arrhythmias or hypokalemia. Serious allergic reactions such as angioedema or anaphylaxis may occur, and if symptoms arise, the medication should be discontinued immediately, and treatment sought. If there is a urinary tract infection, it should be treated first, and the safety in patients with neurogenic bladder has not yet been established. Caution is needed when driving or operating machinery due to drowsiness or blurred vision effects. Concomitant use with other anticholinergic medications may enhance effects or increase side effects, so caution is advised. Additionally, it may reduce the effects of certain medications that promote gastrointestinal motility. This medication is metabolized by specific enzymes in the liver, and when used together with certain strong enzyme inhibitors, the drug concentration may increase, necessitating dosage adjustments. Patients with impaired liver or kidney function should not take this medication with potent enzyme inhibitors. Safety has not been established in pregnant women, so careful consideration is necessary when administering it. It is advisable for nursing women to avoid using this medication. Safety and efficacy in pediatric patients have not been established, so it should not be used in children.
