This medication is used for surface anesthesia of the skin during needle insertion or superficial surgical procedures. It is also utilized for surface anesthesia of the genital mucosa, for instance, prior to the removal of condylomas, and is employed for surface anesthesia at the site of leg ulcers before cleaning or superficial surgery. In other words, it helps reduce pain and discomfort through local anesthesia of various areas, aiding effective treatment.

Dosage and Administration

Adult dosage

1. Skin: After applying this medication, it should be covered with an occlusive tape. The occlusion generally needs to remain for 1-3 hours, and the effect lasts at least 1 hour after tape removal. 1) For needle insertions (e.g., intravenous catheterization, blood sampling) apply about 2g of this medication for a minimum of 1 hour and a maximum of 5 hours. 2) For superficial minor surgical procedures [e.g., removal of seborrheic keratosis], apply 1.5-2g/10cm2 for 1 hour.

Precautions

This medication is used as a local anesthetic and can cause a rare blood disorder called methemoglobinemia. Care should be taken, particularly in patients with glucose-6-phosphate dehydrogenase deficiency, congenital methemoglobinemia, heart or lung damage, infants under 6 months of age, and when used in conjunction with oxidizing agents. The symptoms of methemoglobinemia, such as blue discoloration of the skin or abnormal blood color, should be closely monitored when using this medication. If symptoms appear, use should be immediately discontinued, and appropriate treatment should be sought, which may include oxygen therapy or specialized treatment. Additionally, this medication should not be used in patients with hypersensitivity to it, or in areas with skin injuries, mucosa, contact dermatitis or atopic dermatitis. It should also not be administered to infants under 3 months or to infants under 12 months who are taking medications that induce methemoglobinemia, nor to premature infants born before 37 weeks of gestation. Caution is required for patients with anemia or liver disease, the frail, or elderly patients, and those with a history of allergy to any components of this medication. Local reactions such as redness, swelling, and itching may occur, and rarely, severe allergic reactions or anaphylaxis may arise. It should not be used in sensitive areas like the eyes or middle ear, and if contact with the eyes occurs, they should be rinsed immediately with clean water. In cases of overdose, central nervous system and cardiovascular side effects may occur, requiring careful monitoring, and immediate treatment may be necessary if symptoms are severe. Pregnant or breastfeeding women should carefully consider the benefits versus risks when using this medication. The safety in children, especially infants under 3 months, has not been adequately established, so caution is required, and it is best not to apply it to genital mucosa. The risk of methemoglobinemia may increase when used in conjunction with other drugs, so always consult with healthcare professionals regarding concomitant medications. This medication should be used strictly according to recommended doses and duration, and it is safest to avoid high doses or long-term use.
