Treatment of essential hypertension and heart failure (including ACE inhibitor intolerance), reduction of risk of death after myocardial infarction
This medication helps in controlling blood pressure in patients with essential hypertension and is used in heart failure patients (NYHA Class II to IV) who are intolerant to ACE inhibitors. It is also effective in reducing the risk of death in stable patients with left ventricular dysfunction or decreased contractility after myocardial infarction. This drug plays an important role in the management of hypertension and heart failure, as well as in the prevention of complications after myocardial infarction, improving the patient's health status.
1. Essential Hypertension The recommended dose of valsartan is 80 mg taken orally once daily. If blood pressure is not well controlled, the dose may be increased up to 160 mg once daily or a diuretic may be combined, with a maximum dose of 320 mg per day. Elderly patients (aged 75 years and older) should start with 40 mg once daily. Dose adjustments may be required for patients with renal impairment, dehydration, or liver impairment, and should not exceed 80 mg per day for patients with severe liver impairment. 2. Heart Failure For adult patients, the initial dose is recommended as 40 mg orally twice daily, which can be increased to 80 mg or 160 mg per dose. When combined with a diuretic, a reduction in the diuretic dose should be considered. The maximum daily dose should be divided and does not exceed 320 mg. Triple combination with ACE inhibitors and beta-blockers is not recommended. 3. Reduction of Risk of Death after Myocardial Infarction For adults, treatment can be initiated within 12 hours after myocardial infarction. The recommended initial dose is 20 mg orally twice daily, which can be gradually increased over several weeks to 40 mg, 80 mg, and 160 mg twice daily based on patient tolerance. Consider dose reduction if symptoms of hypotension or renal failure appear. This medication can be used in patients receiving other treatments after myocardial infarction (e.g., thrombolytics, acetylsalicylic acid, beta-blockers, and statins), but combination with ACE inhibitors is not recommended. 4. Patients with Renal Impairment: For patients with a creatinine clearance of 10 mL/min or higher, dose adjustment is not necessary, but this medication should not be used in patients with creatinine clearance below 10 mL/min or in dialysis patients due to lack of experience. 5. Patients with Liver Impairment: For patients with mild to moderate liver impairment, a daily dose of this medication should not exceed 80 mg. If doses above 80 mg are used, it should be considered whether the benefits of therapy outweigh the risks. This medication should not be used in those with severe liver impairment, cirrhosis, or cholestasis. 6. Pediatrics: Safety and efficacy in pediatric patients under 18 years of age have not been established.

This medication can cause serious harm to the fetus if taken during pregnancy, especially in the 2nd and 3rd trimesters, so it should be discontinued immediately upon confirmation of pregnancy. Women who are pregnant or may become pregnant, and women who are breastfeeding should not take this medication. Patients with severely impaired liver function or cirrhosis should not use this medication safely. Patients who have had hypersensitivity to this medication or have experienced allergic reactions such as angioedema with similar medications should not take it. Patients with severely impaired kidney function or certain kidney diseases should also not take this medication. Elderly patients or those with high potassium levels or weakened liver function should be cautious when taking this medication, and dose adjustments may be necessary. Patients with heart disease or cerebrovascular disease should be careful to prevent blood pressure from dropping too low. Low blood pressure may occur upon initial use of this medication, so patients, especially those with fluid or sodium depletion, should be monitored while gradually increasing the dose. When used in conjunction with other antihypertensive medications or diuretics, blood pressure, kidney function, and electrolyte levels should be frequently checked. Caution should be taken when used with potassium-sparing diuretics or potassium supplements as it may lead to dangerously high potassium levels in the blood. Taking this medication with non-steroidal anti-inflammatory drugs (NSAIDs) may reduce blood pressure control and worsen kidney function, so caution is needed. This medication can cause dizziness or drowsiness, so care should be taken when driving or operating machinery. In the event of an overdose, symptoms such as low blood pressure, decreased consciousness, and shock may occur, and immediate emergency treatment is required. The safety and efficacy of this medication in children have not been established, and its use is not recommended. This medication should be stored out of reach of children, and transfer to other containers should be avoided.
