This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. Additionally, it helps alleviate atherosclerotic symptoms such as cardiovascular death, myocardial infarction, and stroke in patients with acute coronary syndrome (unstable angina or non-ST-elevation myocardial infarction) who undergo drug treatment or coronary interventions, including coronary artery bypass surgery. Furthermore, it reduces the risk of ischemic stroke and thromboembolism in adults with atrial fibrillation who have vascular risk factors and for whom vitamin K antagonists are not appropriate and have a low risk of bleeding. Therefore, this medication is effectively used for the improvement of various cardiovascular diseases and for reducing risk.

Dosage and Administration

Adult dosage

1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, take Clopidogrel 75 mg orally once a day. 2. For patients with acute coronary syndrome (unstable angina or non-ST-elevation myocardial infarction), start with a loading dose of 300 mg orally once a day on the first day of treatment, followed by a maintenance dose of 75 mg orally once a day. Aspirin 75 to 325 mg should be taken in conjunction once a day.

Precautions

This medication prevents the clumping of platelets in the blood, thereby preventing clots from forming. People who have a genetic reduction in the enzyme CYP2C19 that activates the drug in the liver may have diminished effectiveness of this medication; in such cases, it is advisable to consult a doctor to adjust the treatment method. Additionally, taking this medication along with drugs that increase CYP2C19 enzyme activity may raise the concentration of the active ingredient of this medication, increasing the risk of bleeding, so it is better to avoid using them together. Patients who have an allergy to this medication or its components, have bleeding issues, have severe liver dysfunction, are lactating women, or have specific genetic conditions such as lactose intolerance should not take this medication. If there is a high risk of bleeding or if bleeding-related symptoms are suspected, seek medical attention immediately, and be particularly mindful as the risk of bleeding may be heightened when combined with other antiplatelet agents or anticoagulants. Patients scheduled for surgery should stop taking this medication 5 to 7 days before the procedure and should use it with caution if they have diseases or conditions that elevate bleeding risk. During the use of this medication, it may take longer for bleeding to stop, so if wounds or bleeding areas persist, inform medical personnel immediately. Rarely, thrombocytopenia or serious blood anomalies may occur; thus, if fever or symptoms of infection appear, inform a doctor. This medication may have side effects such as gastrointestinal disturbances, headache, dizziness, and rash, with bleeding being one of the most significant side effects. Especially for the elderly, and patients with reduced kidney or liver function, the risk of bleeding may be higher, so dosing adjustments and careful monitoring according to a doctor's instructions are essential. During pregnancy, it should be used only if necessary after considering its effects on the fetus, and nursing mothers should consult with a doctor about whether to continue breastfeeding due to the potential secretion of the drug into breast milk. In case of overdose, the risk of bleeding may significantly increase, and as there is no specific antidote, immediate hospital visit for appropriate treatment is necessary. This medication should be kept out of the reach of children, and it is better not to transfer it to another container. Before taking this medication, inform your doctor or pharmacist of any other medications you are currently taking, particularly those that may raise the risk of bleeding.
