Effective for blood sugar control in adult type 2 diabetes and prevention of cardiovascular events
This medication helps control blood sugar levels in adults with type 2 diabetes using empagliflozin and metformin together. In particular, when used in conjunction with dietary control and exercise therapy, it can effectively manage blood sugar levels. Furthermore, this medication is used cautiously in type 2 diabetes patients who have cardiovascular disease, considering the risk of cardiovascular events when blood sugar is not adequately controlled. Thus, it can be used as a therapeutic agent to aid in both blood sugar improvement and cardiovascular health management.
The recommended dose of this medication is 1 tablet, twice a day. The dosage should be determined based on each patient's treatment regimen, efficacy, and tolerability, ensuring that the total does not exceed empagliflozin 25 mg and metformin 2000 mg. For patients using metformin alone or in combination with other medications, the recommended dose is empagliflozin 5 mg twice a day (total 10 mg) along with the existing metformin dose. If tolerance is good and additional blood sugar control is required, the dose may be increased to empagliflozin 25 mg (total daily amount). For patients switching to this medication from a regimen that includes empagliflozin (total daily amount of 10 mg or 25 mg) and metformin, the existing doses of empagliflozin and metformin should be administered. When this medication is used in conjunction with sulfonylureas or insulin, consideration should be given to reducing the doses of sulfonylureas or insulin to decrease the risk of hypoglycemia (see precautions regarding adverse reactions and interactions). This medication should be taken with meals to reduce gastrointestinal side effects associated with metformin. If a dose is missed, it should be taken as soon as remembered, but not taken as a double dose; in that case, the missed dose should be skipped. Special populations: Patients with renal impairment Dose adjustment is not necessary for patients with mild renal impairment. Since this medication contains metformin, it may be used with dose adjustment in patients who have an estimated glomerular filtration rate (eGFR) of 45-59 ml/min/1.73m2, provided there are no other symptoms that would increase the risk of lactic acidosis. In these patients, the starting dose of metformin is 500 mg or 850 mg once daily, so treatment should start with individual agents of empagliflozin and metformin. The maximum daily dose of metformin hydrochloride that can be administered with this medication is 1000 mg, which can be taken as 500 mg twice daily. Kidney function should be carefully monitored every 3-6 months (see precaution for which patients should be used cautiously). If CrCl < 45 ml/min or eGFR < 45 ml/min/1.73m2, this medication must be discontinued immediately (see precaution on which patients should not be administered). Elderly patients Patients over 75 years old should consider the risk of decreased renal function and fluid volume.
This medication helps control blood sugar levels. However, caution is necessary as there is a risk of rare but serious side effects, such as severe lactic acidosis. When used with other diabetes treatments, there is a risk of hypoglycemia, so care should be exercised. This medication is not to be used in patients with poor kidney function, acute kidney problems, serious infections, or heart failure. Patients scheduled for surgery or undergoing tests that use iodine contrast agents should temporarily stop using this medication. It is safest for pregnant or breastfeeding women not to take this medication. This medication may be affected by kidney function; therefore, regular kidney function tests should be conducted. Symptoms of hypoglycemia may occur, so blood glucose levels should be closely monitored, especially when taken with insulin or other blood sugar-lowering agents. Urinary tract infections or genitourinary infections may occur; if symptoms related to these arise, consult a doctor immediately. Patients at risk of reduced fluid volume, such as those with dehydration, diarrhea, or vomiting, should be especially cautious when taking this medication. Vitamin B12 levels may decrease, so patients on long-term treatment should regularly check their vitamin B12 levels. Alcohol intake may increase the risk of lactic acidosis, so excessive drinking should be avoided during treatment. If during the course of treatment any unusual symptoms like fever, genital pain, edema, or rash appear, patients should inform their healthcare provider immediately. Elderly patients may require dose adjustments and regular monitoring due to the risk of renal function decline and decreased fluid volume. In cases of overdose, blood dialysis may be necessary, so dosages must be strictly followed. This medication is not recommended for children. During treatment with this medication, caution is advised when driving or operating machinery, depending on symptoms. Finally, regular consultations with a healthcare provider while taking this medication are important for monitoring health status.
