Treatment of herpes zoster and genital herpes, and suppression of recurrence
This medicine is used to treat herpes zoster virus infections and genital herpes infections, as well as to prevent recurrences.
1. Treatment of herpes zoster Adults: Administer famciclovir 250 mg per dose orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of an initial episode of genital herpes Adults: Administer this medicine 250 mg per dose orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer this medicine 125 mg per dose orally twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this medicine 250 mg per dose orally twice daily. Discontinue treatment periodically every 6~12 months to assess any change in the natural course of the disease. 5. Patients with renal impairment: Because clearance of this medicine is reduced in patients with renal impairment, use caution when adjusting the dose. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and an initial episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose twice daily 10~29 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 125 mg per dose twice daily 10~29 125 mg per dose once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose twice daily 10~29 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. A 4-hour hemodialysis session reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.
This medication should not be used in patients with a history of allergic reactions to this drug, its components, or its active ingredient, penciclovir. Additionally, since this drug contains lactose, it should not be administered to patients with a genetic disorder of lactose intolerance. This medication should be used cautiously in patients with impaired renal function, and dose adjustments may be necessary. Symptoms such as headache, dizziness, nausea, vomiting, abdominal pain, and diarrhea may occur during treatment, and rarely, serious side effects such as thrombocytopenia or renal function deterioration have been reported. This medication does not completely cure genital herpes, so it is important to avoid sexual contact and maintain safe sexual practices when symptoms are present during treatment. If dizziness, drowsiness, or confusion occurs during use, driving or operating machinery should be avoided. Patients with mild or moderate liver impairment do not require special dose adjustments, but caution is advised in patients with severe liver disorders as efficacy may decrease. There are few significant interactions with other medications, but consultation with a doctor or pharmacist is necessary when co-administering with certain drugs due to potential changes in drug concentration or risk of side effects. Women who are pregnant or breastfeeding have not had the safety of this medication fully confirmed, so it should be used cautiously and only when necessary. Children under 18 years of age should not use this medication as its safety and efficacy have not been established. Elderly patients generally do not require dose adjustment, but dosage should be adjusted in the case of impaired renal function. In case of overdose, immediate visit to a medical institution is required for appropriate treatment, and blood dialysis can reduce drug concentration. This medication should be stored in a tightly sealed container to prevent exposure to air or moisture, in a cool, dry place at temperatures below 30 degrees Celsius, and should be kept out of reach of children, avoiding transfer to other containers.
