Relief of neuropathic pain; treatment of epilepsy (partial seizures)
This medication can be used by patients with epilepsy to treat partial seizures either alone or in combination with other therapies. It is particularly effective for simple or complex partial seizures in adolescents aged 13 and older, and can also be used for partial seizures in children aged 3 and older as an adjunctive therapy. Additionally, it helps alleviate neuropathic pain. This drug is used for controlling epileptic seizures and relieving neuropathic pain.
Epilepsy, monotherapy and adjunctive therapy in patients aged 13 years and older: To achieve a maintenance dose of 900 mg/day as gabapentin, titrate the dose over the first 3 days of treatment as follows. Day 1: Administer one 300 mg capsule of this medicine once daily, or one 100 mg capsule of this medicine 3 times daily (gabapentin 300 mg/day) Day 2: Administer one 300 mg capsule of this medicine twice daily, or two 100 mg capsules of this medicine 3 times daily (gabapentin 600 mg/day) From Day 3: Administer one 300 mg capsule of this medicine 3 times daily, or three 100 mg capsules of this medicine 3 times daily (gabapentin 900 mg/day) Alternatively, one 300 mg capsule of this medicine may be administered 3 times daily as the starting dose (gabapentin 900 mg/day). The dose may be increased to 1,200 mg/day as gabapentin, and the total daily dose must not exceed 2,400 mg as gabapentin. (There are insufficient data on the safety and efficacy of doses exceeding 2,400 mg/day as gabapentin.) Neuropathic pain, adults (18 years and older): To achieve a maintenance dose of 900 mg/day as gabapentin, titrate the dose over the first 3 days of treatment as follows. If necessary, the dose may be increased to 1,800 mg/day as gabapentin within one week, and the total daily dose must not exceed 3,600 mg as gabapentin. The total daily dose of this medicine should be divided into 3 doses. Debilitated patients: In debilitated patients with low body weight or patients following transplant surgery, the gabapentin dose should be increased in increments of 100 mg. Elderly patients: Dose adjustment may be necessary due to age-related decline in renal function (see table). Patients with renal impairment (creatinine clearance ≤80 mL/min) or patients receiving hemodialysis: Dosage adjustment is required based on the following table. Dosage for patients with renal impairment: ≥80 mL/min: 900∼2,400 mg/day 50∼79 mL/min: 600∼1,800 mg/day 30∼49 mL/min: 300∼900 mg/day 15∼29 mL/min: 150b∼600 mg/day <15 mL/min: 150b∼300 mg/day a: The total daily dose should be divided into 3 doses. b: 3 × 100 mg should be administered every 2 days. In patients with creatinine clearance below 15 mL/min, the dose should be reduced in proportion to creatinine clearance. Dosage for patients on hemodialysis Initial dose: 300∼400 mg of gabapentin is recommended. Maintenance dose: Administer 200∼300 mg of gabapentin after 4 hours of hemodialysis; do not administer this medicine on days when hemodialysis is not performed. This medicine should be swallowed whole with sufficient fluids and may be taken with or without food. When administering 3 times daily, ensure that the interval between doses does not exceed 12 hours to prevent recurrence of seizures. If a dose of this medicine is missed (if 12 hours or more have elapsed since the last dose), the decision whether to administer an additional dose to make up for it should be made by a physician. When used concomitantly with antacids containing magnesium or aluminum, administer this medicine at least 2 hours after the antacid. If treatment with this medicine is to be discontinued, or if it is to be used concomitantly with another antiepileptic drug, changes should be made gradually over at least 1 week. The safety and efficacy of treatment for neuropathic pain for more than 5 months have not been evaluated. To reduce the main adverse effects, such as somnolence, dizziness, fatigue, and ataxia, the first dose on the first day of treatment should be taken at bedtime.
Children aged 3 to 12 years, first day: Administer gabapentin 10 mg/weight (kg) Children aged 3 to 12 years, second day: Administer gabapentin 20 mg/weight (kg) Children aged 3 to 12 years, from the third day: Administer gabapentin 30 mg/weight (kg); if necessary, the dose may be increased to a maximum of 40 mg per kg of body weight per day. The total daily dose of this medicine should be divided into 3 doses.
This medication is an anti-epileptic drug, and during treatment, there may be changes in mental health such as suicidal thoughts or actions, and depression, which should be carefully monitored. Abruptly stopping this medication may worsen seizures or trigger a persistent state of epilepsy, so it must be discontinued only under a doctor's guidance. In animal studies, there have been reports of carcinogenicity related to pancreatic cancer, but the effects in humans are still unclear. Unexpected and sudden deaths have been reported during the use of this medication, so caution is particularly necessary for epilepsy patients. If symptoms of severe pancreatitis, such as hemorrhagic pancreatitis, occur, discontinue use immediately and consult with medical personnel. Children aged 3 to under 12 may show central nervous system adverse reactions, such as emotional instability, aggression, abnormal thoughts, and hyperactivity, so caution is advised. When taken with central nervous system depressants, including narcotics, there is an increased risk of sedation, respiratory depression, coma, and death, so they should only be administered at minimum doses for a short duration when necessary, and patients must be closely monitored. Individuals allergic to this medication or its ingredients, or those with acute pancreatitis or generalized tonic-clonic seizures should not use it. Patients with impaired kidney function, elderly patients, those with a history of psychosis, pregnant and nursing women, and children with attention deficit hyperactivity disorder should use this medication with caution. Symptoms such as drowsiness, dizziness, and ataxia may occur during treatment, so caution should be practiced when driving or operating machinery. Side effects such as weight gain, visual disturbances, skin rash, and suicidal thoughts may occur, so immediate consultation with healthcare professionals is necessary if any adverse effects are noticed. During pregnancy, this medication may impact the fetus, so those planning to conceive or who are pregnant must consult with healthcare professionals, and caution is also required during breastfeeding. There may be drug interactions with other medications or alcohol, so all medications being taken must be disclosed to the healthcare provider. In cases of overdose, symptoms such as dizziness, drowsiness, and coma may occur, which is why it's critical to adhere to the prescribed dosage, and immediate medical attention is required in the event of an overdose. Store this medication out of reach of children and keep it in its original container to prevent misuse.
