Elderly patients (75 years or older) should start with 40 mg once daily. For patients with renal impairment and creatinine clearance of 10–20 mL/min, start with 40 mg once daily.

Precautions

This medication can cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters; therefore, it should be discontinued immediately if pregnancy is confirmed. It is safe to avoid this medication in patients with certain conditions, such as those with severe liver dysfunction, severe kidney impairment, pregnant women or women who may become pregnant, and individuals allergic to this drug or its components. Care should be taken when using this medication in elderly patients, patients with high potassium levels, patients with mild liver dysfunction, and those with heart disease, as side effects such as excessive lowering of blood pressure may occur; therefore, dosage adjustments and monitoring of the patient's condition should be done gradually. While taking this medicine, symptoms like headache, dizziness, cough, fatigue, and diarrhea may occur, and rarely serious allergic reactions, kidney function deterioration, or blood-related abnormalities may arise, so any unusual symptoms should be reported to healthcare professionals immediately. Particularly, this medication can cause a rapid drop in blood pressure; hence, caution should be observed for symptoms of hypotension when starting treatment or increasing the dose, and necessary measures should be taken as per the physician’s advice. Moreover, taking potassium-sparing diuretics or potassium supplements can lead to dangerously high potassium levels in the blood; thus, regular blood tests to check potassium levels are necessary when used together. This drug may interact with other antihypertensive agents or certain analgesics and anticoagulants, so if other medications are being taken, it is crucial to inform the doctor or pharmacist. Women who are pregnant or breastfeeding should avoid this medication; if it must be taken, breastfeeding should be discontinued. This drug is not recommended for use in children or adolescents under the age of 18 since its safety and efficacy have not been established. In cases of overdose, severe hypotension or loss of consciousness may occur, necessitating prompt medical treatment, and generally, dialysis is not effective. Finally, this medication should be kept out of reach of children, and transferring it to other containers is not advisable for maintaining quality.
