How to take

Adult dose

This medication is generally taken twice a day with meals, and the dosage is gradually increased to minimize gastrointestinal side effects caused by metformin. 1. For patients not previously treated with metformin: The initial dose is 50/500 mg, taken as 1 tablet twice a day, with possible increase to 50/1000 mg as 1 tablet twice daily. 2. For patients currently on metformin treatment: The recommended initial dose is sitagliptin 50 mg twice daily (total daily dose of 100 mg) alongside the current metformin dosing.

Precautions

This medication contains sitagliptin and metformin, which help in blood glucose control for type 2 diabetes. Firstly, this medication can cause rare but severe allergic reactions due to the sitagliptin component, so if symptoms such as skin rash, difficulty breathing, or swelling of the face or neck occur within 3 months of administration, discontinue use immediately and consult a doctor. Additionally, serious side effects such as pancreatitis, which is inflammation of the pancreas, have been reported. If symptoms like abdominal pain, nausea, or vomiting occur, visit a hospital immediately. Although rare, the metformin component can cause a serious metabolic disorder called lactic acidosis, so if symptoms like sudden fatigue, muscle pain, difficulty breathing, or abdominal pain appear, it is important to stop taking the medication immediately and inform healthcare personnel. This medication should not be given to patients with significantly impaired kidney function, severe infections, dehydration, or heart failure. Particularly, before and after tests using radiocontrast iodine, it is safe to stop taking this medication at least 48 hours prior to the test and resume only after confirming that kidney function is normal. It is recommended that this medication not be used by women who are pregnant or may become pregnant, or by women who are breastfeeding. Also, irregular eating, extreme exercise, or the use of other medications can increase the risk of hypoglycemia or lactic acidosis, so caution is necessary. During the course of treatment with this medication, regular monitoring of kidney function, blood tests, and vitamin B12 levels is required, and excessive alcohol consumption should be avoided as it may increase the risk of lactic acidosis. If surgery is approaching, discontinue taking this medication 48 hours before the procedure and only restart after confirming normal kidney function post-surgery. Taking this medication in conjunction with other antidiabetic agents may increase the risk of hypoglycemia, so be alert for signs of hypoglycemia and adjust the doses of other medications if necessary. Finally, if severe joint pain or skin blistering occurs while taking this medication, inform your doctor immediately and seek treatment.
