Improvement of essential hypertension
Essential hypertension is a condition where blood pressure is persistently high without a specific cause, and medications are used to treat it. The purpose of such treatment is to stabilize blood pressure and prevent damage to major organs such as the heart and kidneys. Patients are advised to consistently take the medication while also improving their lifestyle. With appropriate treatment, essential hypertension can be effectively managed.
The recommended initial dose for adults is 10 mg once daily, regardless of meal timing. If blood pressure is not adequately controlled, the dose may be increased to 20 mg once daily, and if necessary, can be increased to a maximum of 40 mg once daily. The blood pressure-lowering effect appears within 2 weeks after starting treatment, and the maximum effect is usually seen in about 8 weeks. If blood pressure is inadequately controlled with this drug alone, it may be administered in combination with thiazide diuretics such as hydrochlorothiazide. To improve adherence, it is recommended to take it at the same time each day (e.g., in the morning). In patients with potential intravascular volume depletion (e.g., those taking diuretics, especially renal impairment patients), it is advisable to start at a lower dose while closely monitoring their condition.
Safety and efficacy in pediatric patients have not been established.
This medication can cause severe harm or even death to the fetus and newborns when administered to pregnant women in the second and third trimesters, so it must be discontinued immediately upon confirmation of pregnancy. This medication should not be used in patients with allergies to its components, pregnant or breastfeeding women, patients on renal dialysis, those with severe liver dysfunction, biliary obstruction, certain genetic lactose intolerance, and diabetic patients with moderate to severe renal impairment who are taking aliskiren. Patients with reduced blood volume or low sodium should be cautious as initial hypotension may occur when starting this medication, and patients dependent on renal function should be closely monitored for worsening kidney function. There is also a risk of hyperkalemia with this medication, so patients with heart failure or kidney dysfunction should have their serum potassium levels checked regularly. Dizziness may occur while taking this medication, so it is advisable to avoid driving or operating dangerous machinery. Various adverse reactions have been reported with this medication, which are generally mild and temporary, including dizziness, headache, back pain, gastrointestinal disturbances, etc. Serious side effects such as chronic diarrhea with weight loss, chronic malabsorption syndrome, and intestinal vascular edema have been reported infrequently, and immediate consultation with a healthcare professional is necessary if such symptoms appear. This medication may dangerously raise serum potassium levels when taken with potassium-sparing diuretics or potassium supplements, so caution is required if used together. Additionally, when used with other antihypertensives, the blood pressure-lowering effect may increase, so it should be taken according to a healthcare professional's instructions. The use of lithium with this medication may lead to elevated serum lithium levels and toxicity, so it is not recommended to be used together. Taking non-steroidal anti-inflammatory drugs with this medication may worsen kidney function, so caution is necessary. This medication can have serious effects on the fetus during pregnancy, so it is essential to consult a healthcare professional if pregnant or planning to become pregnant. If breastfeeding, this medication may pass into breast milk and cause adverse reactions in infants, so consideration should be given to stopping breastfeeding or the medication. Safety in children has not been established; therefore, it is advisable not to use this medication in children. In elderly patients, the concentration of the drug in the body may increase, so dose adjustment and monitoring are necessary. Overdose may result in symptoms such as hypotension and palpitations; severe symptoms should be reported to a healthcare professional immediately. This medication has been reported to have a low risk of carcinogenicity or genotoxicity even with long-term use, but patients should not arbitrarily stop or change their medication without following their healthcare professional's instructions.
