Adults with bipolar disorder: The recommended initial dosage for monotherapy and adjunct therapy is 15 mg once daily without regard to meals.
Adolescents with schizophrenia (ages 13-17): The recommended dosage is 10 mg once daily without regard to meals, starting with an initial dose of 2 mg, then increasing to 5 mg after 2 days, and then to 10 mg after another 2 days
This medication may increase the risk of suicidal thoughts or behaviors in children, adolescents, and young adults with depression or psychiatric disorders, so the physician should carefully assess the necessity and risks of treatment. After treatment begins, family members and caregivers should closely monitor the patient's condition for any deterioration, suicidal tendencies, or unusual behavioral changes, and consult a physician immediately if abnormalities occur. This medication is not approved for treating pediatric depression and requires careful screening and evaluation for suspected bipolar disorder patients. While rare, very serious symptoms known as neuroleptic malignant syndrome may occur while taking this medication. If symptoms such as fever, muscle rigidity, altered consciousness, or irregular pulse arise, medical personnel should be notified immediately. Long-term use may lead to irreversible tardive dyskinesia; thus, it should be given at the minimum effective dose and shortest duration possible, and the physician should be consulted if symptoms occur. In elderly patients with dementia, there may be an increased risk of cerebrovascular accidents like strokes and mortality, so this medication should not be used. It can affect blood sugar control, so diabetic patients should regularly monitor blood sugar levels and consult a physician immediately if hyperglycemia symptoms appear. Due to the risk of venous thromboembolism, patients with thromboembolic risk factors require careful observation and preventive measures before and during administration. Patients with allergic reactions to this medication or genetic issues such as lactose intolerance should not take it. Patients with orthostatic hypotension, seizure history, impaired judgment or motor skills should use this medication cautiously, avoiding activities such as driving or operating machinery if symptoms of drowsiness or dizziness occur. There may be diminished thermoregulation, hence excessive exercise or exposure to high temperatures should be avoided, and proper management should be maintained. Caution should be exercised in patients at risk for dysphagia or aspiration pneumonia. When used in conjunction with antidepressants, there may be an increased risk of suicide, so both patients and caregivers should closely monitor symptoms, prescribing the lowest effective dose to prevent overdose. This medication should not be used for treating Attention Deficit Hyperactivity Disorder (ADHD). Common side effects include nausea, vomiting, constipation, headache, dizziness, anxiety, insomnia, drowsiness, fatigue, and changes in appetite, and if symptoms are severe or persist, medical advice should be sought. During treatment, movement-related side effects such as akathisia, tremor, and extrapyramidal symptoms may occur, so symptoms should be monitored. Long-term use may result in weight gain, so weight changes should be checked regularly and adjusted as needed. Changes in heart rate or EKG abnormalities may occur, requiring careful monitoring in patients with cardiac conditions.
