This medication is used, along with dietary management and exercise, to assist in blood glucose control for patients with type 2 diabetes. Initially, it can be administered alone, but if the patient has never taken any other diabetes medication and controlling blood sugar is difficult with monotherapy, it is used in conjunction with metformin. Additionally, when blood glucose control is inadequate with monotherapy using sulfonylureas, metformin, thiazolidinediones, or insulin, this medication is added. Furthermore, if the effects of combining these medications are still insufficient, this medication is added to improve therapeutic effects. It is used flexibly in various situations to help better control blood sugar.

How to take

Adult dose

This medication is administered at a dose of 100 mg once daily for monotherapy or combination therapy, with a maximum daily dose of 100 mg. This medication can be taken regardless of meals. In patients with renal impairment, dose adjustments may be necessary based on kidney function. For mild renal impairment (eGFR 60–90 mL/min/1.73 m2) and moderate renal impairment (eGFR 45–60 mL/min/1.73 m2), dose adjustments are necessary.

Precautions

This medication may cause severe hypersensitivity reactions after administration; therefore, if allergic symptoms or skin rashes occur, discontinue use immediately and consult a doctor. Additionally, acute pancreatitis has been reported, so if symptoms of pancreatitis such as abdominal pain or vomiting occur, immediate medical attention should be sought. This medication should not be used in patients with type 1 diabetes or diabetic ketoacidosis, and it is also contraindicated in those with allergies to this medication or its components. Patients with impaired kidney function may have increased drug concentration in the blood, necessitating dose adjustments; regular kidney function tests are crucial. Hypoglycemic symptoms may occur while taking this medication, particularly when used in combination with insulin or sulfonylureas, so glucose levels should be monitored closely. Severe joint pain or bullous skin diseases may occur; if these symptoms arise, consult a doctor immediately and consider discontinuing the medication. It is advisable for pregnant or nursing women to avoid using this medication, and safety has not been established for children and adolescents under 18 years of age. In the case of overdose, general supportive measures should be taken, and some medications can be removed through hemodialysis, so treatment should follow medical personnel's instructions if necessary. This medication may interact with some drugs, so inform your doctor or pharmacist if you are taking other medications and follow their guidance. Elderly patients may have a higher chance of kidney function decline, therefore dose adjustments and monitoring of kidney function are needed.
