Treatment of symptoms of overactive bladder (e.g., frequent urination and urinary urgency)
This medication is used to treat symptoms of overactive bladder including urgency urinary incontinence, frequent urination, and urinary urgency. It helps to control excessive contractions of the bladder muscle, alleviating the symptoms of sudden urges to urinate and the frequency of urination. For patients with urgency urinary incontinence, it reduces urinary leakage and helps improve inconveniences in daily life. It alleviates symptoms of frequent urination, reducing the need to get up frequently at night and aiding in sound sleep. Additionally, it prevents the sudden urge to urinate caused by urinary urgency. This medication effectively treats symptoms of overactive bladder by improving overall bladder function.
The usual recommended dose for adults (including older adults) is 5mg once daily. If necessary, the dose may be increased to a maximum of 10mg once daily. This medicine may be taken with or without food and should be swallowed whole with water. Dosage adjustment is required for the following patients. 1. Patients with renal impairment No dose adjustment is necessary in patients with mild or moderate renal impairment (creatinine clearance>30ml/min). Caution is required in patients with severe renal impairment (creatinine clearance≤30ml/min), and the dose should not exceed 5mg once daily. 2. Patients with hepatic impairment No dose adjustment is necessary in patients with mild hepatic impairment. Caution is required in patients with moderate hepatic impairment (Child-Pugh class B), and the dose should not exceed 5mg once daily. 3. Patients receiving potent cytochrome P450 3A4 (CYP3A4) inhibitors For patients receiving ketoconazole or another potent CYP3A4 inhibitor (e.g., ritonavir, nelfinavir, itraconazole), limit the maximum daily dose to 5mg.
This medication should not be used in patients with urinary retention symptoms. Patients with severe gastrointestinal diseases such as toxic megacolon should also avoid taking this medication. It should not be administered to patients with myasthenia gravis or narrow-angle glaucoma, nor in cases of known allergy to this medication or its components. The use of this medication is prohibited in patients undergoing hemodialysis, those with severe liver dysfunction (Child-Pugh grade C), and certain patients with severe renal impairment. Patients with genetic metabolic disorders such as galactose intolerance, lactase deficiency, or glucose-galactose malabsorption should avoid taking this medication. Patients at risk of urinary retention due to bladder outlet obstruction, those with gastrointestinal obstruction, those at risk of reduced gastrointestinal motility, patients with severe renal impairment (creatinine clearance ≤ 30ml/min), moderate liver impairment (Child-Pugh grade B), and those on strong CYP3A4 inhibitors should use this medication with caution. Also, caution is advised for patients with heat hernia, gastroesophageal reflux or those taking medications that may exacerbate esophagitis, and patients with autonomic neuropathy. Common side effects when taking this medication include dry mouth and constipation, as well as blurred vision or dry eyes. Serious side effects such as significant gastrointestinal problems or angioedema may occur in some patients, and symptoms should be discussed with healthcare professionals immediately. This medication may cause cardiac side effects in patients with QT prolongation syndrome or hyperkalemia, so caution is needed. Allergic reactions such as angioedema or anaphylaxis should lead to immediate discontinuation of the drug and appropriate treatment. Caution is advised as this medication may affect driving or operating machinery due to drowsiness, fatigue, or blurred vision. This medication may have enhanced effects or increased side effects when taken with other anticholinergic drugs. It may also reduce the effectiveness of medications that increase gastrointestinal motility, hence caution is needed when concomitantly used. This medication is metabolized by the CYP3A4 enzyme, therefore taking powerful CYP3A4 inhibitors may increase drug concentration and the risk of side effects. In such cases, dose adjustments are necessary, and combination should be avoided in patients with severe renal or liver impairment. The safety of this medication during pregnancy has not been established, so pregnant women should be cautious when taking it, and breastfeeding women should ideally avoid its use. Safety and efficacy in pediatric patients have not been established, so it should not be used in this population. In case of overdose, symptoms may include headache, dry mouth, dizziness, drowsiness, and blurred vision, and appropriate medical attention should be sought immediately. Gastric lavage or activated charcoal may be necessary in cases of overdose.
Not everyone experiences these, and they can vary from person to person.
This medication is sensitive to air and moisture, so it should be stored in a tightly closed container. The storage temperature should be maintained between 1 degree and 30 degrees Celsius to ensure the longevity of the medication's efficacy and safety.
