Summary

Main use
Treatment of herpes zoster and genital herpes, and suppression of recurrence
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer famciclovir 250mg per dose orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer this drug 250mg per dose orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer this drug 125mg per dose orally 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this drug 250mg per dose orally 2 times daily. Discontinue treatment periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Since creatinine clearance of this drug is decreased in patients with renal impairment, dose adjustment should be undertaken with care. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose 2 times daily 10~29: 250 mg per dose 1 time daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose 2 times daily 10~29: 125 mg per dose 1 time daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose 2 times daily 10~29: 125 mg per dose 2 times daily 4) If only the serum creatinine level is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Four hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%; therefore, administer this drug immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.

Precautions

This medication should not be used in patients who are allergic to this medication or its components, or in patients with genetic diseases that impair lactose digestion. Patients with impaired kidney function should use this medication with increased caution, and dosage adjustments may be necessary based on individual needs. Side effects such as headaches, dizziness, drowsiness, nausea, vomiting, and skin rashes may occur during treatment; medical consultation is advised if symptoms are severe. Since this medication does not completely cure genital herpes, it is important to avoid sexual contact and maintain safe sexual relations when symptoms are present. Dizziness or drowsiness may occur when driving or operating machinery, so caution should be exercised in case these symptoms arise. Patients with slightly impaired liver function do not require special dosage adjustments; however, patients with severe liver dysfunction need to exercise caution as the efficacy may be reduced. There are generally few interactions with other medications, but caution is required as this medication's blood concentration may change when used in combination with certain drugs. Women who are pregnant or may become pregnant should carefully consider the benefits and risks of using this medication; similarly, breastfeeding mothers should also make careful judgments. Use in children under 18 years of age is not advised due to lack of established safety and efficacy. Elderly patients typically do not require dosage adjustments, but dosage may need to be adjusted in cases of decreased kidney function. In case of overdose, appropriate treatment is necessary, and hemodialysis may be used to decrease the concentration of the active ingredient in the blood. This medication should be stored in a cool, dry place below 30 degrees Celsius, in a tightly sealed container, and kept out of the reach of children. Transferring to another container should be avoided.
