Treatment of overactive bladder symptoms (frequency, urgency, etc.)
This medication is used to treat the main symptoms of overactive bladder, including urgency incontinence, frequent urination, and urinary urgency. It relaxes the bladder muscles, alleviating symptoms and helping to improve the quality of life for patients. It effectively controls symptoms, thereby reducing discomfort. Thus, it is actively used to improve symptoms of overactive bladder.
The usual recommended dose for adults (including older adults) is 5mg once daily. If necessary, the dose may be increased to a maximum of 10mg once daily. This drug may be taken with or without food and should be swallowed whole with water. Dosage adjustment is required for the following patients. 1. Patients with renal impairment: No dosage adjustment is necessary for patients with mild or moderate renal impairment (creatinine clearance>30ml/min). Caution is required in patients with severe renal impairment (creatinine clearance≤30ml/min), and the dose should not exceed 5mg once daily. 2. Patients with hepatic impairment: No dosage adjustment is necessary for patients with mild hepatic impairment. Caution is required in patients with moderate hepatic impairment (Child-Pugh class B), and the dose should not exceed 5mg once daily. 3. Patients taking strong cytochrome P450 3A4 (CYP3A4) inhibitors: For patients taking ketoconazole or another strong CYP3A4 inhibitor (e.g., ritonavir, nelfinavir, itraconazole), limit the maximum daily dose to 5mg.
This medication should not be used in patients with urinary retention symptoms, severe gastrointestinal diseases, myasthenia gravis, or angle-closure glaucoma. Additionally, it should be avoided in those with hypersensitivity to this drug or its components, patients undergoing hemodialysis, and those with severe liver or kidney diseases. Patients with genetic lactase deficiency or galactose intolerance should also avoid this medication. This medication should be used with caution in patients at risk of urinary retention or with obstructive gastrointestinal diseases or risk of decreased gastrointestinal motility. Patients with severe kidney impairment or moderate liver impairment and those taking strong CYP3A4 inhibitors may require dosage adjustments. Patients with gastroesophageal reflux disease or autonomic neuropathy should also be cautious. Common side effects of this drug include dry mouth, constipation, blurred vision, urinary retention, and dry eyes, particularly dry mouth and constipation, which frequently occur. Severe gastrointestinal adverse reactions have also been reported, so immediate consultation with a doctor is necessary if abdominal pain or bowel disturbances occur during treatment. This medication can increase the risk of cardiac side effects in patients with QT prolongation syndrome or hyperkalemia. Severe allergic reactions such as angioedema or anaphylaxis can occur; if symptoms appear, discontinue the medication immediately and seek treatment. This medication may cause drowsiness or blurred vision when driving or operating machinery, so caution is advised. Combining this medication with other anticholinergic drugs may enhance effects or increase side effects, and may reduce the effectiveness of drugs that stimulate gastrointestinal motility. As this medication is metabolized in the liver, taking it with strong CYP3A4 inhibitors may increase blood concentration and the risk of side effects. Its safety during pregnancy has not been established; it should only be used cautiously when necessary. During lactation, this drug may pass into breast milk and harm the baby, so it is better to avoid it. The safety and efficacy in children have not been established, so this medication is not administered to children. In case of overdose, symptoms may include headache, dizziness, drowsiness, and blurred vision; severe cases may require appropriate emergency treatment. No reports have indicated genetic toxicity or tumor formation risks with long-term use, but any unusual symptoms during use should be immediately discussed with a medical professional.
