Treatment of major depressive disorder and anxiety disorders (panic disorder, social anxiety disorder, etc.) and obsessive-compulsive disorder
This medication is used for the treatment of various mental health issues such as major depressive disorder, panic disorder (whether or not accompanied by agoraphobia), social anxiety disorder (social phobia), generalized anxiety disorder, and obsessive-compulsive disorder.
The safety of doses exceeding 20mg per day has not been established. Administer this drug once daily in the morning or evening, without regard to meals. (1) Major depressive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg daily according to the patient's response. If increasing the dose to 20mg, allow an interval of at least 1 week before increasing the dose. Antidepressants generally take 2-4 weeks to produce an effect. Even after symptoms resolve, at least 6 months of drug therapy is required to ensure a therapeutic effect. (2) Panic disorder with or without agoraphobia: The recommended initial dose is 5mg daily for 1 week, followed by an increase to 10mg daily. The dose may be increased to a maximum of 20mg daily according to the patient's response. The maximum effect appears after approximately 3 months. The safety and efficacy of long-term use of this drug for more than 10 weeks in the treatment of panic disorder have not been systematically studied. The physician should periodically reevaluate each patient to determine the need for continued treatment. (3) Social anxiety disorder: The usual dose is 10mg once daily. Improvement in symptoms generally takes 2-4 weeks. Thereafter, the dose may be reduced to 5mg daily or increased to a maximum of 20mg daily, according to the patient's response. Social anxiety disorder is a chronic condition, and continued treatment for 12 weeks is recommended to ensure a therapeutic effect. Long-term treatment of responding patients has been studied for 6 months and should be considered for each patient to prevent relapse. The usefulness of long-term treatment should be reevaluated regularly. (4) Generalized anxiety disorder: The initial dose is 10mg once daily. Depending on the patient's response, the dose may be increased to a maximum of 20mg daily after at least 1 week of treatment. A long-term treatment study administered 20mg daily for at least 6 months to patients who responded to 12 weeks of open-label treatment. The physician should regularly reevaluate the usefulness of long-term treatment with this drug for each patient. (5) Obsessive-compulsive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg daily according to the patient's response. A long-term treatment study administered 10mg or 20mg daily for at least 24 weeks to patients who responded to 16 weeks of open-label treatment. Obsessive-compulsive disorder is a chronic condition; treatment should continue for a sufficient period to ensure symptom recovery. If this drug is used long term, the usefulness and dose should be regularly reevaluated for each patient. 2) Patients with renal impairment No dose adjustment is required in patients with mild or moderate renal impairment. Use with caution in patients with severe renal impairment (CLCR < 30㎖/min). 3) Patients with hepatic impairment The recommended initial dose is 5mg daily for the first 2 weeks. The dose may be increased to 10mg daily according to the patient's response. 4) Discontinuation of treatment When discontinuing this drug, gradually reduce the dose over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. 5) Switching from MAO inhibitors used to treat psychiatric disorders When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this drug, allow an interval of at least 14 days. Conversely, when starting an MAO inhibitor to treat psychiatric disorders, at least 14 days must elapse after discontinuing this drug. 6) Other MAO inhibitors, such as linezolid or methylene blue Monitor patients for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this drug may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or intravenous administration of this drug at a dose of 1mg/kg or less is unclear.
This medication is an antidepressant used to treat psychiatric disorders such as major depression. It can increase the risk of suicidal thoughts or behaviors, especially in children, adolescents, and young adults aged 18 to 24, so a doctor must carefully assess the necessity and risks before administration. This medication should not be combined with certain drugs, particularly monoamine oxidase inhibitors (MAOIs), due to a heightened risk of serious side effects such as serotonin syndrome. Patients with a history of angle-closure glaucoma or glaucoma need to be cautious as this medication can dilate pupils and increase intraocular pressure. Mental state changes such as anxiety, agitation, suicidal thoughts, and aggression should be monitored closely during treatment, with family members or guardians also keeping a close eye on the patient. Abrupt discontinuation may lead to withdrawal symptoms such as dizziness, insomnia, anxiety, and nausea, so the dosage should be decreased gradually following a doctor's instructions. This medication may prolong the QT interval, increasing the risk of cardiac arrhythmias, so an ECG may be necessary for patients with heart disease or those taking related medications. Pregnant or nursing women should consult their doctor as this medication may affect the newborn. It is advisable not to take this medication with alcohol, and caution is required when driving or operating machinery. In cases of overdose, central nervous system and cardiovascular symptoms may occur; immediate medical attention is necessary. Finally, this medication is not recommended for children and adolescents as its safety and efficacy have not yet been established.
