This medication is used to improve nutritional status by providing energy, essential fatty acids, and omega-3 fatty acids to patients who cannot or insufficiently receive nutrition orally or via the gastrointestinal tract.

Dosage and Administration

Adult dosage

Standard dosage is 1.0 to 2.0 g/kg/day of fat (approximately 5 to 10 ml/kg/day). The recommended infusion rate is 0.125 g/kg/hr (approximately 0.63 ml/kg/hr), and it should not exceed 0.15 g/kg/hr (approximately 0.75 ml/kg/hr). The infusion should be given via central venous or peripheral venous drip.

Children's dosage

Neonates and infants: initial dosage of 0.5 to 1.0 g/kg/day of fat, gradually increased to 3.0 g/kg/day of fat.

Precautions

This medicine should be used with caution in premature or low-birth-weight infants as it may increase blood free fatty acid levels. It should especially not be used in premature infants with severe respiratory illnesses due to the risk of serious side effects such as pulmonary vascular fat accumulation. This medication should not be administered to patients who have allergies to fish, eggs, soybean, peanut proteins, or its components, severe hyperlipidemia, hepatic failure, coagulopathy, renal failure, or acute shock. The use is also prohibited in patients with certain conditions such as acute pulmonary edema, excess fluid, or heart failure. Administration should be done cautiously in patients at risk of lipid metabolism disorders such as diabetes, renal failure, pancreatitis, liver dysfunction, hypothyroidism, and sepsis, as well as those at risk of hyperlipidemia or premature infants. During administration, platelet count, liver function tests, and serum triglyceride levels must be regularly monitored. Adverse reactions such as changes in blood pressure, dyspnea, loss of appetite, nausea, vomiting, fever, skin rash, urticaria, pain, etc., may occur during the use of this medication. If these symptoms appear or serum triglyceride levels increase, the administration should be stopped immediately or the dosage adjusted. This medication is used as part of a comprehensive non-oral nutritional therapy that includes glucose and amino acids, and it's important to monitor blood glucose and triglyceride levels continuously. Caution is needed as excessive administration can lead to fatty overload syndrome, and if symptoms appear, administration should be stopped. During administration, serum triglyceride concentrations should be monitored daily to ensure they do not exceed 3 mmol/L, and regular blood tests should be conducted. If an anaphylactic reaction occurs, administration must be stopped immediately. Caution is required when mixing this medication with other drugs, particularly with heparin due to potential interactions. The safety for pregnant or nursing women has not been established, so it should only be used when the therapeutic benefits outweigh the risks.
