Adult dose

1. Treatment of herpes zoster Adults: Administer famciclovir 250 mg orally per dose 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of an initial episode of genital herpes Adults: Administer this medicine 250 mg orally per dose 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of an acute recurrent episode of genital herpes Adults: Administer this medicine 125 mg orally per dose twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this medicine 250 mg orally per dose twice daily. Discontinue treatment periodically every 6～12 months to assess any change in the natural course of the disease. 5. Patients with renal impairment: Because creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be undertaken with care. The dosage for these patients is as follows. 1) Treatment of herpes zoster and an initial episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30～59 250 mg per dose twice daily 10～29 250 mg per dose once daily 2) Treatment of an acute recurrent episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30～59 125 mg per dose twice daily 10～29 125 mg per dose once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30～59 250 mg per dose twice daily 10～29 125 mg per dose twice daily 4) If only the serum creatinine is known, creatinine clearance may be calculated using the following equations. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, it is preferable to administer this medicine every 48 hours, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer it immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.

This medication should not be used in patients who are allergic to this medication or its components. Patients with impaired kidney function should use this medication with special caution. Mild symptoms such as headache or nausea may occur while taking this medication, and rarely, abnormal blood reactions such as thrombocytopenia or leukopenia may occur. Nervous system symptoms such as dizziness, drowsiness, and confusion may also occur, so driving or operating machinery should be avoided if these symptoms are present. Gastrointestinal symptoms such as abdominal pain, diarrhea, and vomiting or skin rash and itching are commonly reported. It is important to monitor liver health as liver function abnormalities or, rarely, jaundice may occur. Acute renal failure has been rarely reported in patients with poor kidney function; therefore, dosage should be adjusted considering renal status. This medication does not cure genital herpes, so it is necessary to avoid sexual contact and maintain safe sexual practices even during treatment if symptoms are present. This medication may interact with other drugs, so consultation with a professional is necessary, especially when taking medications that affect renal excretion. Women who are pregnant or breastfeeding should use this medication with caution and it should be used only when the benefits of treatment outweigh the risks. It is not recommended for use in children under 18 years of age as safety and effectiveness have not been established. Elderly patients typically do not require dosage adjustments, but dosage should be adjusted in cases of impaired kidney function. In cases of overdose, appropriate treatment and hemodialysis can be used to lower drug concentration. This medication should be stored in a dry place below 30 degrees Celsius and in a secure location out of the reach of children. Care should be taken as transferring to another container may not be good for maintaining quality.
