Treatment of left ventricular systolic dysfunction (stable chronic heart failure)
This medication is used for the treatment of stable chronic heart failure with reduced left ventricular systolic function. It is mainly administered in conjunction with ACE inhibitors and diuretics, and may also be combined with cardiac glycosides as needed to alleviate symptoms and improve heart function. This can enhance the quality of life for heart failure patients and prevent the worsening of symptoms.
As Bisoprolol Fumarate, it is administered orally once daily. The initial dose is 1.25 mg once daily, and the dosage can be gradually increased to 2.5 mg, 3.75 mg, 5 mg, 7.5 mg, and 10 mg at intervals of more than 2 weeks while monitoring tolerance. The maintenance dose is 10 mg once daily. The recommended maximum dose is 10 mg once daily. During the dosage increase period, blood pressure, heart rate, and other patient conditions and heart failure symptoms should be closely monitored.
Due to the lack of experience in pediatric use, administration is not recommended.
This medication lacks sufficient usage experience in patients with certain heart diseases or severely impaired kidney or liver function, and should be used with caution. Bronchodilators should be used in patients with bronchial asthma or chronic obstructive pulmonary disease, and caution is needed as asthma patients may experience increased airway resistance. This medication can worsen allergic reactions and may mask symptoms in patients with hyperthyroidism; caution is advised. Abrupt discontinuation can risk worsening heart conditions, so it should be tapered off gradually as directed by a physician. It should not be given to patients with diabetic ketoacidosis, severe bradycardia, cardiogenic shock, acute heart failure, and other specific conditions. Women who are pregnant or may become pregnant should avoid using this medication as it can be harmful to the fetus. If nursing, it is advisable to either avoid this medication due to its potential secretion in breast milk or stop breastfeeding. Safety in pediatric patients has not been confirmed, so its use is not recommended. In the elderly, it should be started at a low dose while closely monitoring their condition. This medication can interact with other drugs, so patients must inform their doctor of all medications they are taking, especially when using with calcium channel blockers, central nervous system drugs, insulin, etc. Caution is needed while driving or operating machinery due to individual variations, particularly early in treatment or during medication changes.
