This medication is used when blood sugar control is difficult in patients with type 2 diabetes who are following dietary and exercise regimens. It is particularly helpful in cases where blood sugar is not sufficiently controlled by monotherapy with sulfonylureas or metformin. Additionally, it can replace the combination therapy of sulfonylureas and metformin. This helps to effectively control blood glucose levels, improving diabetes management.

How to take

Adult dose

This medication should be administered once daily before the main meal or breakfast. Even if a dose is missed, do not exceed the prescribed dose with the next administration. This medication is a sustained-release formulation and should be swallowed whole without chewing or crushing. The initial dosage is glimepiride/metformin hydrochloride at 2/500 mg daily, adjusted according to the patient’s blood glucose levels and current medications.

Precautions

This medication may cause severe lactic acidosis or hypoglycemia, thus treatment should be discontinued if kidney function deteriorates rapidly. If symptoms of dehydration, high fever, severe diarrhea, or vomiting occur, temporarily stop taking this medication and consult a physician. Patients with poor kidney function or elderly individuals should undergo kidney function tests before taking this medication and should have regular checks of kidney function during treatment. This medication should not be used in patients with type 1 diabetes, acute metabolic acidosis, severe liver dysfunction, or severe kidney impairment. Before and after examinations using radiocontrast iodine agents, this medication should be discontinued and resumed only after kidney function stabilizes. Use with caution in patients undergoing surgery or those with severe infections, trauma, or malnutrition; temporarily discontinuation may be necessary if needed. Pregnant and breastfeeding women should avoid using this medication, and those planning to become pregnant or who are already pregnant should consult their doctors. This medication has numerous drug interactions; therefore, individuals taking other medications should inform their healthcare provider. Hypoglycemic symptoms may occur during treatment; therefore, if symptoms such as headache, dizziness, severe hunger, or tremors arise, immediate intake of sugar is required, and the doctor's instructions should be followed. This medication is in tablet form and should be swallowed whole without crushing or chewing. It is normal for the tablet shell to be visible in stool, so there is no need for concern. Long-term use may lead to vitamin B12 deficiency, hence regular blood tests are necessary. In case of overdose, severe hypoglycemia or lactic acidosis may occur; immediate medical attention is needed. Excessive alcohol consumption should be avoided while on this medication, as alcohol can increase the risk of side effects. Care must be taken when driving or operating machinery, as hypoglycemia or vision disturbances may occur. Regular blood glucose testing and kidney function assessments should be conducted during treatment to monitor efficacy and safety.
