Treatment for hypercholesterolemia (primary, familial) and reduction of cardiovascular disease risk
This medication is used as an adjunct therapy in patients with hypercholesterolemia and mixed dyslipidemia that are not controlled by diet and exercise alone. It is particularly used in conjunction with other lipid-lowering therapies in patients with homozygous familial hypercholesterolemia, helping to slow the progression of atherosclerosis. Additionally, it is used as an adjunctive dietary therapy for pediatric patients meeting certain criteria with hypercholesterolemia. It is also effective as an adjunct in patients with primary dysbetalipoproteinemia. Furthermore, it helps reduce the risk of stroke, myocardial infarction, and artery revascularization in adult patients with elevated high-sensitivity C-reactive protein and multiple cardiovascular disease risk factors. This medication is effective for reducing the risk of various dyslipidemias and cardiovascular diseases.
The initial dose is 5 mg once daily, which can be taken at any time of day, regardless of meals. The maintenance dose is usually 10 mg once daily, with most patients being adjusted to this dose. The maintenance dose should be appropriately adjusted based on LDL cholesterol levels, treatment goals, and patient response at intervals of 4 weeks or more, and can be increased up to a maximum of 20 mg per day.
The usual dosage range for pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17) is 5 mg to 20 mg once daily. The dosage is adjusted according to the treatment goals for individual patients.
This medication should not be used in certain patients. For example, it should not be given to patients with allergies to its components, patients with active liver disease, patients with muscle disorders, patients with severely impaired kidney function, pregnant or breastfeeding women, and fertile women not using adequate contraception. Additionally, high-dose administration is contraindicated in patients at high risk of muscle-related disorders, which include renal impairment, hypothyroidism, family history of muscle disorders, alcohol abuse, etc. Since this medication contains lactose, it should not be used in patients with lactose intolerance or related genetic disorders. Patients with a history of liver disease or excessive alcohol consumption should repeatedly undergo liver function tests before using this medication. If symptoms such as myalgia, muscle weakness, or myopathy (including myositis) occur, patients should immediately inform their doctor, and blood tests may be necessary to confirm muscle damage. If sudden muscle pain or weakness accompanies fever while taking this medication, the use should be discontinued immediately, and medical consultation sought. This medication may interact with certain drugs; therefore, patients taking other medications should consult their physician or pharmacist. Women who may become pregnant should use appropriate contraception while taking this medication, and if pregnancy is confirmed, they should discontinue use immediately. In the case of overdose, there is no specific antidote, and treatment should be symptomatic; liver function and muscle-related markers should be monitored periodically. This medication can rarely cause severe skin reactions or liver dysfunction; therefore, if symptoms such as rash or jaundice appear, patients should immediately inform healthcare staff. This medication may cause dizziness, so caution is required when driving or operating machinery.
