Improvement of blood sugar control in type 2 diabetes
This medication is used in conjunction with dietary and exercise therapy to help control blood sugar levels in patients with type 2 diabetes. It can be administered alone, and if blood sugar control is challenging with monotherapy, it may be used in combination with other oral hypoglycemics or insulin. Additionally, if blood sugar control is insufficient with sulfonylureas or metformin monotherapy, combination therapy with metformin can be utilized. This effectively helps improve the patient's blood sugar levels.
This medication is administered orally once daily. Start with a dosage of 1 mg of glimepiride once daily, increasing the dosage by 1 mg at intervals of 1-2 weeks as needed. Generally, there are minimal additional effects when doses exceed 4 mg, although some patients may increase to 6 mg or 8 mg. If 4 mg daily fails, consider switching to insulin therapy or using insulin or other oral medications.
This medication should not be used in patients with insulin-dependent diabetes or diabetic ketoacidosis, or those in a state of coma. Patients allergic to this medication, its components, or sulfonylureas should also not take it. It is not recommended for use in patients with severely impaired liver or kidney function as safety has not been established; in such cases, switching to insulin therapy is advised. Women who are pregnant or likely to become pregnant, as well as nursing mothers, should not take this medication. Patients with severe infections, or those undergoing surgery or experiencing severe trauma may require insulin therapy. Care should be taken when gastrointestinal issues such as diarrhea or vomiting are present, as food absorption may be compromised, increasing the risk of hypoglycemia. During the first week of taking the medication, there is a heightened risk of hypoglycemia, necessitating close monitoring of blood sugar levels. Special caution is required for the elderly, those with poor nutritional status, irregular eaters, or individuals whose exercise and food intake do not match. Those consuming alcohol or with impaired kidney or liver function may also respond sensitively to hypoglycemia, so careful intake is necessary. Furthermore, overdosage or abnormalities in thyroid, pituitary, or adrenal function can increase the risk of hypoglycemia. When taking this medication alongside others, potential interactions may occur, thus it is imperative to inform your doctor or pharmacist for proper management. This medication has a blood sugar-lowering effect, which could lead to symptoms of hypoglycemia. Symptoms include headache, severe hunger, nausea, fatigue, drowsiness, anxiety, confusion, visual disturbances, dizziness, weakness, and in severe cases, loss of consciousness or coma. Severe hypoglycemia can present symptoms similar to a stroke, thus immediate treatment is required. Additionally, blurred vision may occur initially, along with gastrointestinal symptoms such as nausea, vomiting, abdominal pain, or diarrhea. Serious side effects such as liver dysfunction or decreased blood cell counts may also occur rarely, so any abnormal symptoms should be reported to a healthcare professional immediately. When taking this medication, it is essential to take it regularly in conjunction with dietary and exercise therapy for optimal blood sugar control. In case of hypoglycemia, it is important to consume sugar or sugary beverages quickly, and it is advisable to always carry sugar. Education on hypoglycemic symptoms and their management should be thoroughly provided to patients and their families. Regular blood sugar and urine tests should be performed during treatment to monitor the drug's efficacy and side effects, and dosage adjustments should be discussed with a doctor if conditions change. Furthermore, caution should be exercised during driving or operating machinery to avoid accidents due to hypoglycemia. As this medication is metabolized in the liver, its interaction with specific drugs may affect blood sugar control. Notably, some antibiotics, antifungals, antihypertensives, and anticoagulants may interact, hence it is crucial to inform healthcare providers about other medications being taken. Alcohol intake can unpredictably affect blood sugar control, necessitating caution, and adjusting the timing of medication intake is also important. Pregnant or breastfeeding women should avoid this medication as it may harm the fetus or newborn. In cases of planned pregnancy, consulting with a doctor to choose alternative methods like insulin therapy is advisable. The safety of this medication has not been adequately established in children and adolescents under 18 years, and therefore, it should not be used as a principle. For the elderly, careful attention is needed due to potentially diminished kidney function, including dosage adjustments. In the event of overdose, severe hypoglycemia may occur, necessitating immediate emergency treatment. Induction of vomiting, gastric lavage, or glucose injection may be required. This medication should be stored in a safe place out of the reach of children, in its original container to prevent misuse or degradation of quality.
