This medicine is used for patients who require parenteral nutrition, specifically when nutritional supply via oral or gastrointestinal routes is impossible or insufficient. This drug helps improve nutritional status by supplying energy, essential fatty acids, and omega-3 fatty acids needed by the patient.

Dosage and Administration

Adult dosage

The dose and infusion rate should be determined based on the ability to excrete fat. It should be administered by drip infusion via central or peripheral veins. The standard dose is fat 1.0-2.0 g/kg/day (approximately 5-10 ml/kg/day). A recommended infusion rate is fat 0.125 g/kg/hr (approximately 0.63 ml/kg/hr), and it should not exceed fat 0.15 g/kg/hr (approximately 0).

Precautions

This drug should be used with caution when administered to premature or low birth weight infants. These patients may have a reduced ability to eliminate lipids from the body, potentially leading to elevated levels of free fatty acids in the blood. Additionally, it should not be administered to patients with allergies to fish, eggs, soybean, peanut proteins, or any components of this drug, as well as those with severe hyperlipidemia, liver failure, blood coagulation disorders, renal failure patients not undergoing filtration or dialysis, patients in acute shock, acute lung edema, those with fluid overload, and heart failure. This drug should be administered cautiously to patients with complications that affect lipid metabolism, such as diabetes, renal failure, pancreatitis, liver dysfunction, hypothyroidism, and sepsis. In particular, premature infants or newborns with hyperbilirubinemia and pulmonary hypertension should have their platelet counts, liver function tests, and blood triglyceride levels monitored regularly. During administration, various adverse reactions such as changes in blood pressure, shortness of breath, loss of appetite, nausea, vomiting, fever, chills, skin rashes, etc., may occur, requiring close observation. If blood triglyceride levels rise above 3mmol/L, administration should be stopped or the dose reduced. This drug is used as part of parenteral nutrition therapy, and regular monitoring of blood glucose and triglyceride levels is important. Overdosing may lead to lipid overload syndrome, so signs of metabolic abnormalities should be closely monitored. During administration, blood triglyceride concentrations should be checked daily, and care should be taken to keep levels below 3mmol/L. Clinical tests such as liver function tests, acid-base balance, and electrolyte concentrations should also be performed regularly. If serious allergic reactions such as anaphylaxis occur, administration should be stopped immediately. The combination of medium-chain and long-chain fatty acids in this drug helps reduce the risk of metabolic acidosis. Therefore, it is recommended to administer it along with carbohydrates. When mixing with other drugs, it is essential to confirm the compatibility of the medicines. This drug contains soybean oil, fish oil, and egg phospholipids, which can rarely cause allergic reactions. Patients with soybean and peanut allergies should be particularly cautious. There is insufficient safety data for pregnant or breastfeeding women, so it should be used only when the therapeutic benefits outweigh the risks. When administering this drug, high levels of blood lipids may affect the results of certain blood tests, so caution should be taken when interpreting test results. Overdose may cause lipid overload syndrome, and in such cases, administration should be stopped immediately, and the dose adjusted if necessary. This drug should be used immediately after opening, and if not used, it should not exceed 24 hours of refrigeration. Before administration, check visually that the emulsion is homogeneous, and it is safe to use immediately even if mixed with other medicines.
