Summary

Main use
Improvement of essential hypertension
Who takes it
Adults · Children
Adult dose
The recommended initial dose is 10 mg once daily, regardless of meals. If blood pressure is not adequately controlled at this dose, it can be increased to 20 mg once daily. If additional blood pressure-lowering effect is needed, the dose can be increased up to a maximum of 40 mg once daily. The blood pressure-lowering effect appears within 2 weeks after starting treatment, with maximum effect observed around 8 weeks. If control of blood pressure is not achieved with monotherapy, it may be combined with thiazide diuretics such as hydrochlorothiazide. To improve adherence, it is recommended to take it at the same time each day (e.g., in the morning). For patients at risk of intravascular volume depletion (e.g., patients on diuretics, especially those with renal impairment), treatment should be started at a low dose with careful monitoring.
Child dose
Safety and efficacy in children have not been established.

View approved original text

Essential hypertension is treated to lower blood pressure reliably, which reduces the strain on the heart and blood vessels and decreases the risk of complications such as stroke or myocardial infarction. This medication controls blood pressure and improves the overall health of the patient.

How to take

If control of blood pressure is not achieved with monotherapy, it may be combined with thiazide diuretics such as hydrochlorothiazide. Precautions

This medication can cause serious harm or death to the fetus and newborn if taken during the second and third trimesters of pregnancy, so it should be discontinued immediately if pregnancy is confirmed. Patients with allergies to any components of this medication, pregnant women, breastfeeding mothers, patients undergoing kidney dialysis, those with severe liver impairment, and patients with obstructive biliary disease should not take this medication. Additionally, patients with lactose intolerance or certain genetic metabolic disorders should not use this medication. Patients with decreased blood volume or low salt levels may experience a sudden drop in blood pressure early in treatment, so careful monitoring is required. Patients with impaired kidney function or kidney transplant recipients should have their kidney function and electrolyte status monitored regularly through blood tests, and it is not recommended for patients with severe kidney impairment. Dizziness may occur due to blood pressure reduction, so caution is needed while performing dangerous activities like working at heights or driving. This medication is used for the treatment of hypertension and is generally safe, but some patients may experience side effects such as dizziness, headache, abdominal pain, muscle pain, and rash. If taken by a pregnant woman, it may cause serious issues such as reduced kidney function in the fetus, reduced skull development, and amniotic fluid deficiency, so it should be avoided during pregnancy. Breastfeeding mothers should consider discontinuing breastfeeding or stopping the medication, as it may be passed into breast milk. Safety in children has not been established, so caution is needed for use in pediatric patients. Elderly patients may require dosage adjustments or monitoring due to slowed drug metabolism. When co-administering with other medications, blood pressure-lowering effects may increase or blood potassium levels may rise, so consultation with a physician is necessary. In case of overdose, hypotension and arrhythmias may occur, necessitating appropriate emergency treatment if needed. This medication has been reported to have a low risk of carcinogenicity or genotoxicity even with long-term use.
