Treatment of hypercholesterolemia (primary, familial) and reduction of cardiovascular disease risk
This medication is primarily used as an adjunct for patients with hypercholesterolemia, particularly those with heterozygous familial hypercholesterolemia and mixed dyslipidemia, when diet and exercise are insufficient. It is also used alongside other lipid-lowering therapies such as LDL separation techniques in patients with homozygous familial hypercholesterolemia. This medication lowers total cholesterol and LDL cholesterol levels to target levels in hypercholesterolemia patients, delaying the progression of atherosclerosis and protecting cardiovascular health. It can also help reduce total cholesterol, LDL cholesterol, and Apo-B protein levels in some pediatric patients aged 10 to 17 years who meet specific criteria. It may also be used as an adjunct to dietary therapy in patients with primary dysbetalipoproteinemia. Additionally, it is used to reduce the risk of stroke, myocardial infarction, and arterial revascularization in specific patients aged over 50 years in men and over 60 years in women who have certain cardiovascular risk factors. In summary, this medication helps improve health by addressing various types of dyslipidemia and reducing the risk of cardiovascular disease.
This medication can be administered at any time of the day, regardless of meals. The initial dose is 5 milligrams once daily, and if further LDL cholesterol reduction is needed, the maintenance dose can be adjusted accordingly. The maintenance dose is usually 10 milligrams once daily, which most patients are adjusted to. The maintenance dose should be appropriately adjusted based on LDL cholesterol levels, treatment goals, and patient response at intervals of 4 weeks or longer.
The conventional dosage range for pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17 years) is 5 milligrams to 20 milligrams once daily. The dosing should be tailored individually based on the recommended treatment goals for each patient.
This medication should not be used in certain patients. For instance, individuals allergic to the components of this medication, those with severely impaired liver function, those with muscle disorders, and those with severely impaired kidney function should not take this medication. Additionally, pregnant or breastfeeding women and women of childbearing age who are not using appropriate contraceptives should also avoid this medication. Patients who may be prone to muscle-related side effects should exercise caution, and high doses of this medication are contraindicated. This medication contains lactose, so individuals with lactose intolerance or related genetic disorders should not take it. Patients taking this medication should undergo regular liver function tests if they consume large amounts of alcohol or have a history of liver disease. Patients whose hypercholesterolemia is caused by hypothyroidism or nephrotic syndrome should treat the underlying conditions first. Muscle-related side effects such as myalgia, muscle weakness, myopathy, and rarely rhabdomyolysis may occur, so if any abnormal muscle symptoms arise during treatment, the physician should be informed immediately. If muscle injury markers are high in blood tests, medication may need to be discontinued. Patients with impaired kidney function or specific diseases should be monitored very carefully, and in cases of acute severe conditions, medication should be discontinued. This medication can rarely cause severe skin reactions, so if rash, itching, fever, or other symptoms appear, it should be stopped immediately and the medical staff consulted. Interactions with other medications may occur, so if other medications are being taken, this must be disclosed to the medical staff. The risk of muscle side effects may increase when used in conjunction with specific antibiotics, antifungal agents, immunosuppressants, or fibrate-class drugs. Pregnant women or those planning to become pregnant should not take this medication, and it should be stopped immediately if pregnancy occurs during treatment. Breastfeeding women are also advised to avoid this medication. In cases of overdose, there is no specific antidote, so treatment should be symptomatic and liver function and muscle damage should be assessed. Caution is advised when driving or operating machinery due to the possibility of dizziness. This medication may have higher systemic concentrations in Asians, necessitating dose adjustments. It is important to consult medical staff immediately if any unusual symptoms or suspected side effects occur during treatment.
Angioedema (including swelling), severe allergic reactions, rhabdomyolysis, severe muscle weakness, ophthalmoplegia, severe skin adverse reactions (Stevens-Johnson syndrome, eosinophilia and systemic symptom drug reaction syndrome), jaundice, hepatitis, fatal and non-fatal liver failure, severe edema, acute kidney failure, oliguria, severe rash accompanied by high fever, blistering, skin peeling, pain in the oral/ocular mucosa, difficulty breathing
