This medication helps improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral artery disease. It is also used to alleviate symptoms such as death due to cardiovascular abnormalities, myocardial infarction, stroke, or refractory ischemia in adult patients with acute coronary syndrome, such as unstable angina or non-Q-wave myocardial infarction who have received drug therapy, coronary intervention, or coronary artery bypass surgery. Additionally, it is prescribed to patients with atrial fibrillation who have one or more vascular risk factors, when anticoagulation with vitamin K antagonists is inappropriate and the risk of bleeding is low, to reduce the risk of thromboembolic complications such as stroke and thromboembolism. In summary, this medication is effectively used for various cardiovascular diseases and thrombus prevention.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral artery disease, clopidogrel should be orally administered at a dose of 75 mg once daily. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), start with a loading dose of 300 mg once daily on the day of initiation and thereafter maintain with 75 mg once daily. In this case, aspirin 75 – 325 mg should be administered once daily along with this medication.

Precautions

This medication helps prevent the aggregation of platelets in the blood to ensure proper circulation. It may be less effective for those who genetically have reduced functionality of a certain enzyme (CYP2C19), and alternative treatment options should be considered for such individuals. Additionally, co-administration of this medication with drugs that activate certain enzymes may increase the concentration of this medication and raise the risk of bleeding, so concurrent use with such medications is not recommended. Do not take this medication if you have an allergy to it or its components, have bleeding, have severe liver damage, are breastfeeding, or have certain hereditary lactose intolerance. Patients with a risk of bleeding or suspected bleeding symptoms should have an immediate blood test, and special caution should be exercised when combining with other antiplatelet or anticoagulant medications. Patients scheduled for surgery should discontinue this medication 5-7 days prior to the procedure and can restart after hemostasis has been achieved. Long-term use of this medication may result in side effects such as bleeding, reduction of blood cell counts, gastrointestinal disturbances, and skin rashes, so any unusual symptoms should be discussed with a doctor. Women who are pregnant or breastfeeding should consult with a specialist when taking this medication as it may affect the fetus or newborn, particularly in the case of delivery when there is a risk of bleeding, necessitating discontinuation. This medication is not recommended for use in children, as its safety and efficacy have not been established. Elderly patients or those with impaired kidney or liver function may have a higher risk of bleeding and require dosage adjustments and careful observation. Overdose of this medication can lead to prolonged bleeding time and severe hemorrhage, so immediate medical attention is necessary in case of overdose, as no specific antidote exists; treatments such as platelet transfusion may be required. This medication should be kept out of reach of children, and transferring it to another container is not advisable for quality maintenance.
