Adult dose
For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. In patients with acute coronary syndrome, start with a loading dose of 300 mg once daily on the day of initiation and continue with a maintenance dose of 75 mg once daily thereafter. At this time, aspirin should be co-administered at 75 to 325 mg once daily. For patients with atrial fibrillation, administer 75 mg orally once daily.

This medication is used to improve symptoms in adult patients with atherosclerotic conditions such as ischemic stroke, myocardial infarction, and peripheral artery disease. It helps reduce the risk of myocardial infarction, stroke, and death due to cardiovascular anomalies in patients with acute coronary syndrome. Additionally, in patients with atrial fibrillation who have a low risk of bleeding and for whom vitamin K antagonist therapy is not appropriate, it is effective in reducing the risk of stroke and thromboembolism. This drug is used to improve symptoms and risks in patients with various cardiovascular risks.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. In patients with acute coronary syndrome, start with a loading dose of 300 mg once daily on the day of initiation and continue with a maintenance dose of 75 mg once daily thereafter. At this time, aspirin should be co-administered at 75 to 325 mg once daily. For patients with atrial fibrillation, administer 75 mg orally once daily.

Precautions

This medication inhibits platelet aggregation to prevent blood coagulation. Individuals with a genetically diminished function of a specific enzyme (CYP2C19) may experience reduced effectiveness of this medication, and alternative treatment options should be considered in such cases. Taking this medication with drugs that activate CYP2C19 enzymes may increase the blood concentration of this medication and raise the risk of bleeding; therefore, it is best to avoid concomitant use. Patients who are allergic to this medication or its components, those with active bleeding, severe liver impairment, nursing mothers, and individuals with specific genetic disorders such as lactose intolerance should not take this medication. Patients at high risk of bleeding, those with impaired liver or kidney function, recent stroke patients, patients with hypertension, elderly patients, and underweight individuals should use this medication with caution and discontinue use 5 to 7 days before any scheduled surgeries. This medication can cause bleeding; therefore, if any symptoms of bleeding occur, it should be reported to healthcare professionals immediately, especially if taken with other bleeding risk medications. Regular monitoring through blood tests for bleeding or abnormal blood reactions is necessary. This medication can, in rare cases, cause thrombocytopenia or severe blood disorders; consult healthcare professionals immediately if symptoms of fever or infection arise. Gastrointestinal symptoms (abdominal pain, dyspepsia, diarrhea, etc.) or side effects such as rash, headache, and dizziness may occur. During pregnancy, safety data for the fetus is limited and is administered based on a physician’s judgment; use may be discontinued before delivery due to increased risk of bleeding. In nursing mothers, since this medication may be secreted into breast milk, consultation with a doctor regarding whether to continue or discontinue breastfeeding is essential. Elderly patients are at increased risk of bleeding due to decreased bodily function, necessitating dose adjustment and monitoring of their condition. Overdosage may increase the risk of bleeding, as there is currently no antidote for this medication, and severe bleeding may require treatment such as platelet transfusion. This medication is not recommended for children, as safety and efficacy have not been established. This medication may interact with various other medications, so if you are taking other drugs, you must inform your healthcare provider and be cautious about concurrent use with medications that pose a bleeding risk. While taking this medication, the time for bleeding to stop may be prolonged, so report any bleeding symptoms to healthcare professionals immediately, and be sure to inform your medical personnel about this medication before any surgery or dental treatments.
