Adult dose
For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, the recommended oral dose of Clopidogrel is 75 mg once daily. For patients with acute coronary syndrome, begin with a loading dose of 300 mg once daily on the initiation day and maintain with 75 mg once daily thereafter. This medication is helpful in improving symptoms in adult patients with atherosclerotic symptoms such as ischemic stroke, myocardial infarction, and peripheral arterial disease. It is also used to improve cardiovascular complications (such as myocardial infarction and stroke) in patients with acute coronary syndrome, particularly those with unstable angina or non-Q-wave myocardial infarction who are receiving or will receive drug treatment, coronary interventional procedures, or coronary artery bypass surgery. Additionally, it is effective in reducing the risk of stroke and thromboembolism in patients with atrial fibrillation who have vascular risk factors and where vitamin K antagonists are not suitable and the bleeding risk is low. This medication aids in reducing major cardiovascular risks and preventing thrombosis, thereby improving the patient's health status.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, the recommended oral dose of Clopidogrel is 75 mg once daily. For patients with acute coronary syndrome, begin with a loading dose of 300 mg once daily on the initiation day and maintain with 75 mg once daily thereafter. Aspirin 75 to 325 mg must be given once daily in conjunction at this time. For patients with atrial fibrillation, the recommended oral dose is 75 mg once daily.

Precautions

This medication is designed to prevent platelets in the blood from aggregating, thereby preventing the formation of blood clots. This medication must be converted to its active form in the liver for effectiveness; thus, individuals with genetically reduced function of the liver enzyme CYP2C19 may experience weakened efficacy. Therefore, in such cases, it is necessary to consult with a physician to adjust the treatment approach or to switch to another medication. Moreover, concomitant use with other medications that activate the CYP2C19 enzyme may increase the active ingredient concentration of this medication and elevate the risk of bleeding, so combination should be avoided. Patients who have allergies to this medication, those with severe bleeding, severe liver diseases, breastfeeding women, and those with lactose intolerance should not take this medication. If symptoms of bleeding occur during treatment, immediate consultation with a doctor is essential, especially for patients at risk of bleeding in delicate areas such as the gastrointestinal tract, brain, and eyes. Concomitant use with other platelet inhibitors or anticoagulants such as aspirin may further increase the risk of bleeding, thus caution is advised. Patients scheduled for surgery should discontinue use of this medication 5–7 days prior to the surgery and recommence only after hemostasis is achieved. This medication may rarely cause hematologic adverse reactions such as thrombocytopenia and neutropenia, so it is imperative to seek immediate consultation with a physician if symptoms of fever or infection arise. Mild side effects such as dyspepsia, diarrhea, headache, and rash may occur, and in cases of severe bleeding or allergic reactions, immediate medical attention is necessary. The risk to the fetus during pregnancy is not clearly defined, however, treatment should not be delayed for those with cardiovascular and cerebrovascular diseases, and caution is warranted as the risk of bleeding may increase during delivery. In breastfeeding cases, as this medication may be secreted into breast milk, the option to either cease breastfeeding or to discontinue the medication should be chosen. Elderly individuals or patients with impaired kidney or liver function are at increased risk of bleeding, hence careful observation and dosage adjustment should be undertaken. In cases of overdose, the risk of bleeding increases, and since there is no specific antidote, treatment such as platelet transfusion may be required in cases of severe bleeding. Store the medication out of reach of children and do not transfer it to another container. During treatment, any new medication initiation or surgery should be communicated to healthcare providers and any symptoms of bleeding should be reported immediately.
