Treatment of genital herpes and suppression of recurrence; treatment of herpes zoster
This medication is used to treat herpes zoster virus infections and genital herpes infections, and is also effective in suppressing the occurrence of recurrent genital herpes. Through this, it alleviates symptoms and reduces viral activity to aid recovery.
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir per dose orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of first-episode genital herpes Adults: Administer 250 mg of this medication per dose orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer 125 mg of this medication per dose orally 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medication per dose orally 2 times daily. Discontinue treatment periodically every 6~12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Because clearance of this medication is reduced in patients with renal impairment, dose adjustment should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and first-episode genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose 2 times daily 10~29: 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose 2 times daily 10~29: 125 mg per dose once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose 2 times daily 10~29: 125 mg per dose 2 times daily 4) If only serum creatinine is known, creatinine clearance may be calculated using the following equations. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is preferable, taking the interval between dialysis sessions into account. A 4-hour hemodialysis session reduces the plasma concentration of this medication by approximately 75%; therefore, administer this medication immediately after dialysis. The recommended dose is 250 mg for patients with herpes zoster and 125 mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.
This medication should not be used by patients who have had specific allergic reactions or have genetic issues such as lactose intolerance. Caution should be exercised when administering this medication to patients with impaired kidney function. Symptoms such as headache, nausea, dizziness, and drowsiness may occur with this medication; rare side effects such as decreased blood cell count or skin rashes have also been reported. Since this medication does not completely treat genital herpes, it is important to avoid sexual contact and maintain safe sexual relations when symptoms are present. If dizziness or drowsiness occurs while taking this medication, operating a vehicle or machinery should be avoided. Patients with moderately decreased liver function do not require special dosage adjustments, but those with severe liver disease need to be cautious as the medication's efficacy may be reduced. This medication has little significant interaction with other drugs, but blood concentration may change when used in conjunction with certain medications, so consultation with a doctor is necessary. Women who are pregnant or breastfeeding should carefully consider the benefits and risks of using this medication. Safety and efficacy have not been established for children under 18 years, so use in this age group is not recommended. Elderly patients may require dosage adjustment based on kidney function. In cases of overdose, appropriate treatment and hemodialysis may be beneficial. This medication should be stored in a tightly closed container to prevent exposure to air or moisture. It should be kept in a cool, dry place below 30 degrees Celsius and out of reach of children. Transferring to another container is not advisable as it can be detrimental to maintaining quality.
