This medication is used to improve symptoms associated with atherosclerosis related to ischemic stroke, myocardial infarction, and peripheral arterial disease. It also helps reduce the risk of death from cardiovascular abnormalities, myocardial infarction, stroke, and refractory ischemia in adult patients with acute coronary syndrome or those who have undergone related procedures. Additionally, it plays a role in reducing the risk of stroke and thromboembolism in patients with atrial fibrillation who have a low risk of bleeding, where vitamin K antagonist therapy is not appropriate. This medication is used to enhance patient health by improving risk factors due to various cardiovascular diseases and lesions.
1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, the recommended dose is 75 mg orally once daily as clopidogrel. 2. For patients with acute coronary syndrome (unstable angina or non-ST elevation myocardial infarction), start with a loading dose of 300 mg once orally on the day of initiation, followed by a maintenance dose of 75 mg orally once daily. During this period, aspirin 75 to 325 mg should be administered concurrently once daily.
This medication is an antiplatelet agent that prevents platelets from aggregating, thus preventing blood clotting. Before taking this medication, it is essential to verify if there are individuals with genetically reduced functionality of a specific enzyme (CYP2C19). Individuals with impaired enzyme activity may not respond properly to this medication, potentially experiencing cardiovascular issues such as heart attacks or strokes more frequently. Additionally, using this medication along with drugs that activate the enzyme may lead to excessively high drug concentrations, increasing the risk of bleeding, so concurrent use is not recommended. This medication should not be used in individuals with certain diseases or allergies, those with bleeding issues, patients with very poor liver function, lactating women, and individuals with lactose intolerance. Patients with a risk of bleeding or a tendency to bleed should use this medication with caution, and it should be discontinued 5-7 days prior to any scheduled surgery. During treatment, if any bleeding symptoms (e.g., bruising, nosebleeds, gum bleeding) occur, medical personnel should be notified immediately. When taken with other drugs that increase bleeding risk, anticoagulants, nonsteroidal anti-inflammatory drugs, or selective serotonin reuptake inhibitors (SSRIs), the risk of bleeding may increase significantly, so caution is needed. During pregnancy, the safety of this medication for the fetus has not been adequately established; however, treatment should not be delayed in emergencies like myocardial infarction or stroke. If lactating, careful consideration should be given to whether to continue breastfeeding or to stop medication, as this drug can be excreted in breast milk. Elderly patients or those with impaired renal or liver function have a higher risk of bleeding, so dosage adjustment and careful monitoring according to a physician's instructions are necessary. In case of overdose, prolongation of bleeding time and bleeding complications may occur, requiring immediate treatment. Although there is no specific antidote for this medication, platelet transfusions can counteract the effect. This medication should be kept out of reach of children and should not be transferred to another container. While it is known that this medication does not significantly affect driving or operation of machinery, caution should be exercised if bleeding symptoms occur.
