Acute coronary syndrome, symptoms of atherosclerosis, and reduction of thrombotic risk in atrial fibrillation
This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. It also helps in improving atherosclerotic symptoms such as death from cardiovascular events, myocardial infarction, and stroke after medication treatment, coronary interventions, or coronary artery bypass surgery in patients with acute coronary syndrome, particularly those with unstable angina or non-Q-wave myocardial infarction. Additionally, it is used to reduce the risk of all types of atherothrombotic and thromboembolic events in adult patients with atrial fibrillation who have one or more vascular risk factors, where vitamin K antagonists are not appropriate and there is a low bleeding risk.
○ Adults 1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), start on the day of initiation with a loading dose of 300 mg of this medicine once daily, followed by a maintenance dose of 75 mg orally once daily. In this case, aspirin 75 ~ 325 mg should be administered once daily in combination with this medicine. 3. For patients with atrial fibrillation, administer 75 mg of this medicine orally once daily. In this case, aspirin 75 ~ 100 mg should be administered once daily in combination with this medicine. ○ Patients with renal impairment: Treatment experience in patients with renal impairment is limited. ○ Patients with hepatic impairment: Treatment experience in patients with moderate liver disease and a bleeding diathesis is limited. This medicine may be taken with or without food.
This medication prevents platelets from aggregating and helps prevent blood clotting. The effectiveness of this drug may be reduced in patients genetically deficient in a specific enzyme, CYP2C19, so these patients should reconsider their treatment options. Furthermore, when taken with drugs that activate CYP2C19, the active ingredient concentration of this medication may increase, raising the risk of bleeding; therefore, concomitant use should be avoided. It should not be administered to patients with allergies to this drug, those with bleeding disorders, individuals with severe liver damage, breastfeeding women, and patients with certain genetic lactose intolerance issues. Patients at high risk of bleeding or those suspected of having bleeding symptoms should frequently be monitored by blood tests during the use of this medication, and caution is especially required when used with anticoagulants like warfarin. For patients scheduled for surgery, it is advisable to discontinue this medication 5 to 7 days prior to the surgery and to resume it once the bleeding risk has decreased. Also, when used together with drugs that increase bleeding risk, such as aspirin, non-steroidal anti-inflammatory drugs (NSAIDs), thrombolytics, or certain antidepressants, it should be taken cautiously under the guidance of a physician. During the course of this medication, it may take longer for bleeding to stop; therefore, if there are any sites of bleeding or if bleeding persists for a long time, medical personnel should be notified immediately. Rarely, thrombocytopenia or severe blood disorders may occur, and testing may be necessary during symptoms of infection or fever to consider possible bone marrow dysfunction. Gastrointestinal symptoms may include abdominal pain, indigestion, and diarrhea, but they are generally mild. Skin reactions such as rashes or itching have also been reported, so if there are any abnormalities in the skin, consultation with a physician is necessary. In pregnant women, while no fetal risks have been reported, the risk of bleeding during delivery may increase, so care should be taken during use, and breastfeeding mothers should consider whether to discontinue breastfeeding due to the possibility of the drug being excreted in breast milk. Elderly patients or those with impaired kidney or liver function have a higher risk of bleeding, thus requiring dose adjustment and monitoring of their condition. Overdose may increase the risk of bleeding, and while there is no specific antidote, platelet transfusion may be necessary to stop bleeding. This medication should be kept out of the reach of children and should not be transferred to other containers as this may adversely affect quality.
