View approved original text

This medication is used for patients with type 2 diabetes who struggle to control blood sugar levels with diet and exercise alone, particularly overweight adult patients. Additionally, this medication can be used as monotherapy and, if necessary, can be combined with other oral hypoglycemic agents or insulin. It can also be used in children over 10 years and adolescents in growth as either monotherapy or in combination with insulin. This medication helps effectively control blood sugar to assist in diabetes management.

How to take

Adult dose

The dosage should be determined individually based on drug efficacy and tolerance, and should not exceed the recommended maximum dosage per day. Dosing should start at a lower dosage, typically 500 mg taken 2-3 times daily with meals. Dosage increases should be made gradually by 500 mg weekly, and doses up to 2000 mg should generally be divided and taken twice daily in the morning and evening. If doses exceed 2000 mg, they should be taken 3 times daily.

Child dose

Precautions

This medication should not be administered to patients with impaired kidney function. Serious side effects like lactic acidosis may occur if kidney function is moderately or severely impaired. This medication should not be used in patients with acute conditions such as acute infections, heart failure, or myocardial infarction. In such states, the risk may increase due to worsened kidney function. Patients who are scheduled to undergo examinations using iodine-based contrast agents should temporarily discontinue this medication for a certain period before and after the exam, and resume it only after confirming normal kidney function. This medication is not administered to patients who have had allergic reactions to it, patients with type 1 diabetes, or those with diabetic ketoacidosis. Patients with severe infections, significant trauma, or those undergoing surgery should temporarily stop taking this medication, and can resume it only after their condition has stabilized and kidney function is confirmed to be normal. Patients with poor nutritional status or those with decreased liver function, and patients who consume excessive alcohol should use this medication cautiously. Liver impairment may increase the risk of lactic acidosis. This medication can lead to a rare but life-threatening side effect called lactic acidosis, so patients and caregivers should immediately inform their doctor if symptoms of lactic acidosis such as muscle pain, difficulty breathing, fatigue, or abdominal pain occur. Gastrointestinal symptoms (diarrhea, nausea, vomiting, etc.) may occur initially, but they are usually temporary and improve over time. Taking the medication slowly and with meals can reduce symptoms. While this medication lowers blood sugar, it rarely causes hypoglycemia on its own. However, caution is needed as hypoglycemia may occur when taken with other hypoglycemic agents, when meals are skipped, or during intense exercise. Regular monitoring of kidney function, blood sugar, and vitamin B12 levels is recommended to ensure there are no issues. Particularly, elderly patients and those with decreased kidney function require more frequent monitoring. This medication may be used during pregnancy only under special circumstances and with a physician's judgment, but should not be used in pregnant women at risk of placental insufficiency or preeclampsia. If breastfeeding, a discussion with a doctor is necessary regarding whether to continue breastfeeding or to discontinue medication, as this medication can pass into breast milk. This medication may interact with other drugs, so it is essential to inform the doctor or pharmacist of any other medications being taken, and to closely monitor blood sugar and the patient’s condition. In case of overdose, the medication can be removed via hemodialysis, so if overdose is suspected, immediate medical attention is necessary. This medication should be stored out of reach of children, and it is advised to avoid transferring it to another container, as it can negatively affect the quality.
