Treatment of hypertension
This medication is used alone or in combination with other antihypertensive agents such as beta-blockers or diuretics to effectively treat hypertension. It helps to stabilize blood pressure, preventing complications related to hypertension and improving the patient's health.
1. Adults The therapeutic dose of this medicine should be adjusted appropriately according to the patient's response and the severity of symptoms. The initial dose is lacidipine 2 mg once daily. Administer it at the same time each day, preferably in the morning, regardless of meals. If an increase is needed, increase to 4 mg once daily after an appropriate period has elapsed to allow the pharmacological effect to be adequately assessed. Increase to 6 mg once daily only if blood pressure is not controlled with 4 mg once daily. The period before deciding to increase the dose should generally be at least 3 ~ 4 weeks, the time needed for this medicine to achieve its full pharmacological effect, except in patients requiring more rapid dose escalation. If an adequate response is not achieved, another antihypertensive medicine may be added (a beta-blocker and/or a diuretic). 2. Patients with hepatic impairment Because this medicine is primarily metabolized in the liver, its bioavailability and blood pressure-lowering effect may be increased in patients with hepatic impairment. These patients should be monitored carefully, and a reduction in the initial dose is recommended. 3. Patients with renal impairment This medicine is not excreted through the kidneys; therefore, dose adjustment is not necessary in patients with renal impairment. 4. Older adults The initial dose is 2 mg once daily. If necessary, the dose may be increased to 4 mg once daily after 4 weeks.
There is no experience of using this drug in children.
This medication should not be administered to certain patients. For example, patients with an allergy to this medication or similar medications, pregnant or breastfeeding women, patients with severely compromised heart function should not use this medication. Patients with cardiogenic shock or unstable angina, patients with aortic stenosis, patients within one month after myocardial infarction, patients with left ventricular dysfunction, and patients currently receiving dantrolene should also not use this medication. When using this medication, caution should be exercised in patients with issues related to the sinoatrial or atrioventricular nodes, patients with low cardiac reserve, patients with liver dysfunction, patients with prolonged QT interval, and patients with unstable angina. While this medication is generally safe, side effects such as dizziness, headache, palpitations, flush, abdominal discomfort, skin rash, and edema may occur. Most side effects are temporary and may disappear with continued use. In particular, patients with cardiovascular diseases may experience worsened angina; if symptoms appear, they should consult a doctor immediately. Abrupt discontinuation of this medication may worsen symptoms, so the dosage should be gradually reduced according to the doctor's instructions, and care should be taken not to stop taking it arbitrarily. In the initial stages of treatment, blood pressure may drop and heart rate may temporarily increase; if these changes occur, the patient should discuss them with their doctor. This medication can have a stronger blood pressure-lowering effect when used with other antihypertensive agents, so potential interactions with other medications being taken should be thoroughly checked. It is advisable to avoid taking this medication with grapefruit juice, as it may alter the serum concentration of the medication. Pregnant or breastfeeding women should refrain from using this medication due to reported risks of uterine muscle relaxation and prolonged pregnancy and delivery delays in animal studies. Safety for pediatric use has not been established, so it should not be administered to children. In the case of overdose, prolonged hypotension and rapid heart rate may occur, and bradycardia or heart conduction disorders may arise; immediate medical assistance should be sought. This medication should be kept out of reach of children and should not be transferred to other containers.
