Improvement of blood glucose control in type 2 diabetes mellitus
This medication is used in conjunction with dietary management and exercise therapy to help control blood sugar levels in patients with type 2 diabetes. It can be administered alone when blood sugar control is needed independently. However, if blood sugar control is insufficient with monotherapy, it is given in combination with metformin, sulfonylureas, or thiazolidinediones to enhance blood sugar control effects. This medication is effectively used to improve blood sugar management in patients.
1. Monotherapy The recommended initial dose is 0.5 mg of repaglinide per dose, with the dose determined through titration at intervals of 1 or 2 weeks. Patients taking another oral hypoglycemic agent may also switch directly to this medicine; in this case, the recommended initial dose is 1 mg per dose. The recommended maximum dose is 4 mg per administration, taken before meals 2, 3, or 4 times daily according to the patient's meal pattern. The total daily dose must not exceed 16 mg. Greater caution should be exercised when administering this medicine and titrating the dose in debilitated or malnourished patients. Although this medicine is excreted mostly via the bile, caution should also be exercised in patients with renal impairment. 2. Combination therapy If blood glucose is not adequately controlled with monotherapy, this medicine may be used in combination with metformin or a thiazolidinedione. The initial dose and dose adjustment are the same as for monotherapy. The doses of each agent should be adjusted very carefully to the minimum required to achieve the desired pharmacologic effect. If dose adjustment is not performed appropriately, the incidence of hypoglycemic symptoms may increase. Appropriate monitoring of fasting blood glucose (FBG) and glycated hemoglobin (HbA1c) should be performed to prevent overdose or an increased likelihood of failure of the second agent.
This medication acts to lower blood sugar and may cause symptoms of hypoglycemia, which is when blood sugar levels become too low. Hypoglycemia can occur more easily during fever, injury, infection, or surgery, so caution is needed. This medication should not be used in certain patients. For example, individuals allergic to this medication or its ingredients, patients with type 1 diabetes who require insulin, patients who are in a coma due to diabetes, women who are pregnant or breastfeeding, children under 18 years old, patients with severe liver dysfunction, and those taking certain medications should not use this medication. Patients with impaired liver or kidney function, those with heart disease or gastrointestinal disorders, individuals with poor nutritional status or irregular eating habits, those who engage in excessive exercise, those who consume excessive amounts of alcohol, and the elderly should use this medication with caution. While taking this medication, symptoms such as hypoglycemia, visual disturbances, gastrointestinal disturbances, liver function anomalies, and allergic reactions may occur. Particularly, hypoglycemia can be easily controlled by carbohydrate intake, but in severe cases, a glucose injection may be required in a hospital setting. This medication interacts with many others, so if you are taking other medications, you must consult with a doctor or pharmacist. Caution is especially required when taken with certain antibiotics, antifungals, cardiac medications, antihypertensives, and specific diabetes treatments, as blood sugar may drop too low or the effectiveness may change. Women who are pregnant or breastfeeding should not use this medication, and children or the elderly should use it cautiously as safety and efficacy have not been sufficiently verified. In case of overdose, symptoms of hypoglycemia such as dizziness, sweating, tremors, and headaches may occur, and if severe, immediate medical treatment should be sought. This medication should be stored out of reach of children, and it is safest to keep it sealed in its original container.
